The Advanced Practice Clinician (APC) provides in-person care four days per week and serves on PPGP's centralized follow-up team one day per week. The APC will function in an expanded role as the primary provider of medical services. This position provides direct care to patients seeking family planning, gender affirming care, and primary care. The APC works to assess the clinical needs of patients, in compliance with our medical standards & guidelines, including ordering, interpreting, and recording results of clinical tests. Prescribes medications and makes recommendations for other forms of treatment, including birth control.

As a member of the follow-up team, the APC works in conjunction with providers across the affiliate to ensure continuity of high-quality patient care by overseeing, executing and managing the follow up plans of care for all services provided at PPGP, thus ensuring an excellent patient experience. 

Responsibilities
    • Essential Job Functions - Direct Patient Care (80%).
    • Provides quality health care services including exams, diagnosis, treatment, education and counseling for clients in accordance with PPGP protocols and Medical Standards & Guidelines.
    • Secures a complete health history, including gynecologic, contraceptive, medical, surgical, sexual, family health, and psychosocial; records findings accurately and succinctly.
    • Performs physical examinations with special emphasis on the reproductive system, including breast examinations, pelvic examination, cancer screening tests, diagnosis of sexually transmitted infections (STIs), and other types of more specialized procedures as may be indicated by medical policy.
    • Performs, orders and interprets diagnostic studies as indicated and permitted by affiliate medical protocols.
    • Consults with Medical Director and other Clinicians to ensure compliance with standards and guidelines.
    • Provides relevant health instruction to include family planning, nutrition, sexual counseling, and principles of health promotion and maintenance.
    • Essential Job Functions - Follow-Up Role (20%)
    • Run daily follow-up report from the electronic medical records system (EMR) to identify patients requiring urgent follow up actions.
    • Address urgent follow-up needs on the follow-up report with a focus on ER Referrals and Early PregnancyComplications.
    • Address non-urgent follow-up plans including abnormal paps, positive syphilis tests, HIV results, and colpo/leep results.
    • Notify patients (phone call/letter) of abnormal test results and initiate treatment.
    • Generate patient letters based on PPGP Centralized Follow-up Team Policy
    • Task to Follow-up Support to mail letters and notify health department of positive test results
    • Collaborate with follow-up nursing when needed, including to schedule a colpo/LEEP or MAB Ultrasound appointment.
    • Work collaboratively with the Centralized Follow Up (CFU) Team and attend CFU team meetings.
    • Perform limited telehealth clinician appointments.
    • Provide clinic coverage of time off for other clinicians.
    • Ensure compliance with PPFA Medical Standards & Guidelines, and all PPGP medical protocols and policies in treatment and referral of patients.
    • Must have completed and submitted all four state applications within 30 days of hire.
Requirements
    • Required Education and Experience
    • An unencumbered multi-State APC license is required for this position; if APC does not currently have MSL, one must be obtained within 30 days of hire. 
    • Previous experience in Family Planning and Women's health.
    • Knowledge of CDC STD treatment guidelines and management of pap abnormalities.
    • Knowledge of management of early pregnancy complications.
    • Preferred Education and Experience
    • Master's degree appropriate for licensure. 
    • Experience in Gender affirming care a plus.

    • While performing the duties of this job, the Nurse Practitioner (NP) is occasionally required to stand; walk; sit; use hands and fingers to handle or feel objects, tools or controls; reach with hands and arms; climb stairs; talk and hear. The employee must occasionally lift and/or move materials that may weigh up to 40 pounds.
    • Use of hands and fingers involved in work which is preformed through the extensive use of computer systems, email, phone and fax machines, copiers and scanners
    • Handwritten paperwork is performed
    • Employee is required to sit and stand for extended periods of time with prolonged exposure to computer monitors
    • Must operate various input/output devices including keyboard and computer mouse.
    • This position carries some level of exposure to conflict and/or antagonism by those opposed to the mission of PPGP.
$54.05 - $58.41 an hour
