LSF is looking for a talented ­­­­­­­­­­­­­Adoptions Case Managerwho wants to make an impact in the lives of others.

Purpose and Impact:

Adoptions Case Managers are intended to provide assistance and support to the Case Management Organization by leading the activities and tasks to promote adoptions for children in the system of care.

$22.84 starting salary

$23.80 if certified in child welfare case management with 1 year of child welfare case management experience.

$24.76 if certified in child welfare case management with 3 years of child welfare case management experience.

Essential Functions:

  • Coordinate and facilitate all adoptions transfer staffing's and activities.
  • Provides adoption expertise and support to case management team to promote timely adoptions of children.
  • Serves as LSF liaison with prospective adoptive families.
  • Links adoptive families to community supports and services.
  • Participate in adoption disclosure staffing's between agency and adoption families.
  • Partner with lead agency to ensure licensed families who wish to adopt have a viable point of contact for questions and concerns.
  • Provide statistical information and feedback to CBC on perspective and closed adoptions.
  • Participate and represent LSF in various recruitment, Heart Gallery, and other CBC sponsored events.
  • Perform placement follow-up contacts with assigned foster homes upon placement of children in those homes.
  • Prepare required reports and other documentation as instructed by supervisor.
  • Represents LSF in community outreach programs and activities which shall include, but are not limited to, recruitment events, foster/adoptive parent meetings and support groups.
  • Attend and participate in staff/professional trainings, meetings, and supervision sessions.
  • Complete 90 days of post finalization contact with all adoptive families to include calls at a minimum of once every 30 days.
  • Maintain monthly tracking log of finalizations for each fiscal year.
  • Work closely with Case Manager to assist with obtaining all needed items to transfer all cases within 30 days of termination of parental rights.

  • Communication Skills: Keep supervisor fully informed of activities, pertinent issues, upcoming events and potential problems. Demonstrate effective oral and written communication skills in daily work.
  • Teamwork: Support the unit, department and/or organization and work with others in an effort to accomplish the goals of the unit, department and/or organization.
  • Safety: Employee makes a reasonable effort to adhere to established safety procedures and practices in the work area
  • Training: Attend and successfully complete all mandated training courses as required
  • Confidentiality: Adhere to all confidentiality rules

Other Functions:

  • Perform other related duties and special assignments as required.
  • Must carry agency cell phone at all times and participate in on-call activities as needed
  • Must demonstrate sensitivity to our service population’s cultural and socioeconomic characteristics and needs.

Physical Requirements:

  • Must have a high level of energy, be adaptable to irregular hours, be able to travel as needed.
  • Valid driver’s license and appropriate auto liability insurance required.

Skills:

  • Excellent written and verbal communication skills
  • Ability to drive both locally and throughout the state in connection with the duties of this position.
  • Solid organizational skills with the ability to maintain updated physical and digital records.
  • Solid knowledge of adoption best practices and recruitment procedures
  • Excellent time management and leadership skills
  • Impeccable interpersonal skills and communication skills
  • Proficiency in spreadsheet software, and various Microsoft applications including Power Point, Adobe, Publisher, and word processing applications.
  • Public speaking with various audiences
  • Ability to partner and collaborate effectively with clients, parents, co-workers, volunteers, governmental and other agencies and the general public.

Education:

A bachelor’s degree in human services is required.

Experience:

Experience working with foster/adoptive families is required. Minimum of two years of experience with child welfare case management also required. Most possess or able to achieve child welfare professional certification within one year of hire.

  • Reports to Child Welfare Supervisor.
  • Team player with co-workers, community partners, and administrators.
  • Accurate and timely submission of required documents and reports.
  • Adherence to all licensing and quality assurance standards.
  • Liaison to government, courts, schools, and community providers.
  • Adherence to agency policies and procedures.
  • Must maintain confidentiality of all client information.
