Description
The Case Manager works with families who have physical or mental disabilities that present barriers to maintaining stable housing. These families reside on-site and receive case management services through our permanent supportive housing program. The individual in this role will work with clients to establish family and personal goals and provide case management. The successful candidate will have excellent customer service, verbal and written communication skills, the ability to deal with challenging situations with tact and professionalism. Must have flexibility with schedule and the ability to work weekends when necessary.
Duties and Responsibilities
• Screen applicants for Project SAFE for eligibility through telephone screening, office visits, shelter visits, and written documentation
• Develop Family Action Plans with clients that prepare them for more independent living
• Secure all the necessary documentation required for HUD to verify family income from employment, entitlement programs, and other sources
• Record units of service and report to supervisor monthly
• Complete Move-In Inspections with the Maintenance Supervisor that homeless families will occupy, do inventory, and provide rent calculations according to HUD guidelines
• Provide appropriate referrals to GED and/or ESOL classes, tutoring, training programs, counseling, legal services, credit counseling, parenting, life skills classes or other community resources
• Provide weekly face-to-face contact with families and visit families in their homes weekly, or as determined with supervisor, to review progress
• Work with families on budgeting monthly
• Arrange transportation for families as needed
• Attend meetings and trainings as assigned
• Attend court, medical, school, therapy and other appointments with and/or for clients as needed
• Collaborate with community providers providing services to families as needed
• Enter all eligible families and needed information into the CMIS system
• Maintain client files, complete required reports, and other paperwork in a timely manner.
• Provide data for program reporting quarterly and as requested
• Monitor the progress of Family Action Plans monthly
• Re-assess families using the SPDAT, or other designated tool, according to Continuum of Care Guidelines
• All other duties as assigned
Requirements
• Bachelor's degree in social work or a related field
• Minimum of 3 years' experience in social work or human services working with homeless/vulnerable populations.
• Must be reliable and have ability to work early evening hours and/or weekends when necessary
• Valid Florida Driver's License with good driving history and ability to be insured
• Familiarity with local support groups and agencies that provide social services
• Excellent verbal and written communication skills
• Proficiency in Microsoft Office (Word, Excel, Outlook)Continue
Preferences for
• Knowledge of Client Track, or other HMIS related software
• Preference for 5 or more years' experience working with housing and/or homeless families.
• Master's degree in Social Work or closely related field
• Experience working with clients with mental disabilities/dual diagnosis

Salary Description
$45,000 Annually
