Admissions Screener | Qualifications:

• Long term care and/or hospital experience preferred.
• The Admissions Screener must possess integrity, poise and diplomacy while demonstrating an ability to interact well with diverse segments of the community, while marketing our health system.
• Good judgement coupled with organizational, communication, and decision-making skills are essential. Proficiency in Word and Excel coupled with basic computer skills is necessary.

Admissions Screener | Role & Responsibilities:

• Be available to screen at hospitals, community facilities, and other referral sources, as directed.
• Complete the on-line Pre-Screen Information and Assessment form on prospective residents.
• Research discharge plans for short-term residents prior to admission.
• Consult with the Director of Admissions and/or other administrative staff regarding candidates for admission.
• Maintain a working knowledge of medications, medical procedures and treatments, and medical terminology.
• Possess a working knowledge of Medicaid, Medicare, and HMO guidelines and protocols in order to transmit this information accurately to residents and/or responsible parties, when necessary.
• Maintain a positive interactive presence at local hospitals and facilities, and with social workers and discharge planners.
• Distribute marketing materials to hospitals, community facilities, senior centers, and other referral sources, as needed and directed.
• Accept other responsibilities and duties as the need arises and as requested by the Director of Admissions or Administrator.
• Attend seminars, workshops, and inservices pertinent to the job, when applicable or required.
• Adheres to dress code policy at all times.
• Adheres to time and attendance policy at all times.
• Practices safe work habits and follows all safety procedures of the facility.
• Assures the safety, security, and wellbeing of our residents

Job Type: Full-time

Pay: $20.95 - $52.06 per hour
