TITLE OF IMMEDIATE SUPERVISOR: DIRECTOR OF BUSINESS DEVELOPMENT

DUTIES OF POSITION
The Clinical Liaison is responsible hospice admissions and for interaction in the community and promoting company services. As well as will provide the following duties including, but not limited to: managing the patient intake/admissions process-in conjunction with the Admissions and Hospice Team. The responsibilities below outline the expectations.
POSITION RESPONSIBILITIES
Serve as Admissions Nurse, admitting appropriate patients onto hospice service.
Complete referral/intake forms and route them appropriately for admission approval.
When necessary, complete coding for all new admissions
When necessary, request, collect and gather any outstanding paperwork necessary to complete referral/admission including but not limited to: orders, dc summaries, H&P, medication lists
Complete initial care plans for new patients
Enter medication lists (when appropriate) for new patients
Complete audits to ensure compliance for new admissions
Provide equipment needs to Intake/Business Office when appropriate
Work in conjunction with DOCS to ensure Clinical Team assignment for new admits and all other needs
Obtain d/c documents; facility face sheet; transport time; location for hospital discharge patients and new admits
F/u with families as necessary or determined by DBD, NP, DOCS, Intake/BO
Conduct and/or support business development team with in-services
Perform other duties as directed.
Care conference support (may include, attending; preparing documents; reviewing medical information engaging appropriate team members)
Facility notification of staffing changes (when appropriate)
Meet with new staff as a part of orientation process
Visit Doctor' offices, hospitals, Assisted Living facilities, Skilled Nursing Facilities and other possible sources of referrals to present Agency credentials and obtain patient referrals.
Review patient/company relationships and implement appropriate sales plan.
Develop sales/marketing objectives and projections.
Develop a marketing plan, identifying priorities and sets a reasonable timetable.
Review and evaluate the analyses and plan on an established basis.
Attend IDG meetings, as appropriate.
Assist office staff as needed with going to Physician’s offices to obtain signatures, drop off IDG updates, etc.
Provide community outreach and education.
JOB CONDITIONS
Position is stressful in terms of meeting deadlines.
Physical activities will include, walking, sitting, stooping, standing and lifting up to 25 lbs.
EQUIPMENT OPERATION
Must be able to use a computer, multi-line telephone, and other office equipment.
COMPANY INFORMATION
Has access to patient medical records.
QUALIFICATIONS
- Must be WI licensed RN in good standing.
- At least 1 year experience in marketing with health agencies/professionals.
- Must be organized and detail oriented.
- Must be able to communicate effectively orally and in writing.
- Must be a licensed driver with an insured automobile in good working order.
Job Type: Full-time
Pay: $72,100.00 - $74,900.00 per year
