The Social Service Admissions Coordinator develops and manages an effective system to gather resident information before and upon admission to assure proper placement of the resident. The Social Services Admission Coordinator works to maintain facility census while determining if facility can provide the anticipated care services of the referral. Education Must have a high school diploma Qualifications 1. Must be able to read and write with good grammar skills. 2. Is self-motivated and able to take the initiative to complete duties as specified and directed by the Administrator. 3. Possesses a valid driver’s license. 4.Demonstrates leadership and managerial ability. 5. Has the ability to organize, plan and supervise resident admissions in coordination with the Administrator. 6. Maintains effective interpersonal relationships and works well with the general public. 7. Is capable of implementing changes as determined by the Administrator. 8.Is of good emotional, mental and physical health having sound judgment and high professional standards. 9. Is courteous in working with residents, families and employees. 10. Maintains a neat, well-groomed and professional appearance at all times. 11. Has a desire to work with the geriatric clientele as well as residents of any age and their current prospective care needs. 12. Must be willing to be in and/or available to the facility weekends as well as weekdays. 13. Must demonstrate ability to follow policies, procedures, practices and direction without the direct supervision of the Administrator. 14. Must demonstrate the ability to function as a cooperative team member with all disciplines in the facility. 15. Must have a pre-employment and an annual Tuberculosis screening examination as required by the state the Social Service Admissions Coordinator is employed.
Responsibilities: The Admissions Coordinator works with the Administrator to assure the following responsibilities are accomplished. This list is only partial and should not be interpreted as a comprehensive inventory of all duties, tasks and responsibilities. 1. Has a working knowledge of resident’s rights and is an advocate for the resident. 2. Has a working knowledge of policies within the Social Service Department and throughout the facility, and abides by these policies. 3. Completes documentation in a timely and professional manner. 4. Communicates with Department Heads and other staff when necessary to insure admissions and readmissions requirements are met. 5. Keeps confidential any and all information regarding the residents’ personal and clinical records. 6. Participates in the community-oriented events and special marketing projects as requested by the Administrator. 7. Maintains confidentiality, adhering to established policies and confidentiality in regards to residents, employees, and family members. 8. Attends required staff in-services. 9. Other duties as assigned by the Administrator. 10. Have the ability to learn computer systems, system applications and related office equipment. 11. Promotes effective work relationships with the administrator, medical staff, personnel, volunteers, agencies, and management staff affiliated with the facility. 12. Gathers sufficient pre-admission data to determine resident level of care and the ability of the facility to properly care for the resident. 13. Completes pre-admission and admission documents and forms per policy. 14. Collects accurate data for all payer source referrals. 15. Interacts with discharge planners in an effective and efficient manner. Ask for appropriate and adequate resident information. 16. Coordinates room assignments for residents in consultation with the nursing department and administration. 17. Assist with admissions, transfers and discharges in accordance to facility policy. 18. Explain the facility admission agreement and all required admission documents to potential and new residents and residents' representatives 19. Works directly with Account Manager on each admission to ensure all financial information is accurately explained. Obtains required signature on all admission forms. 20. Prepares resident identification bands, bracelets and other appropriate identifying labels as required. 21. Acts as a liaison between transferring institutions and the facility; establish a good rapport with transferring institutions. 22. Participates in marketing of the facility and maintaining an acceptable census level. 23. Attends and participates in facility meetings. 24. Participates in Quality Assessment and Improvement Program. 25. Participates in Quality Assessment Performance Improvement Program. 26. Reports immediately to supervisor and administrator situations involving abuse, neglect, or suspicion of a crime. 27. Adheres to the Code of Conduct, Compliance Program, HIPPA Standard, and Computer Must have a high school diploma, Language Skills Ability to read and communicate effectively in English Skills• Analytical skills • Basic computer knowledge• Organizational skills • Ability to lead others in a supervisory capacity
