JOB SUMMARY
Empres Operated by Evergreen is currently seeking an Admissions Nurse to join our team! The Admissions Nurse is directly accountable to the Director of Nursing Services to complete the Nursing Admission process for new admits.
SCHEDULE
- Mon - Sat from 10am to 6pm.
ESSENTIAL FUNCTIONS
- Completes Nursing Admission Evaluations and initiates the care plan, care directives, meal monitor and ADL flow sheets and other documentation forms as appropriate for the individual resident’s plan of care.
- Obtains and records the resident’s weight and height at time of admission.
- Notifies the receiving physician of resident’s arrival to the Center.
- Verifies orders with receiving physician.
- Communicates admission orders in a timely manner to pharmacy.
- Completes the Medication Administration Record and Treatment Administration Record at time of admission and reviews with the unit licensed nurse.
- Completes the day of admit Initial Skin Evaluation, Fall Risk Evaluation, Wandering Evaluation, and Smoking Evaluation, and initiates safety interventions needed.
- May be assigned other clinical tasks by DNS related to Continuous Quality Improvement.

- Strong diplomatic communication skills with residents, responsible parties, staff, and other community persons.
- Highly customer-oriented and have strong attention to detail.
- Demonstrates up-to-date knowledge in current nursing standards of practice.
- Demonstrate strong clinical assessment and technical skills.
- Self-directed, strong organizational and time management skills.
EDUCATION, LICENSURE, AND EXPERIENCE
- Graduate of an approved nursing program
- Valid Oregon state nursing license (LPN or RN)
- One year previous supervisory experience in skilled nursing center
