Full-time position starting at $22.00 per hour; benefits include 3 weeks PTO (prorated), 10 paid holidays, Medical, Dental, Vision, 401K with matching, etc.

Job/Task Functions

The Activities Case Manager works at the Residential Treatment Facility (RTF) to plan, schedule, and provide a wide variety of ongoing activities to all residents. The Activities Case Manager also provides services to residents including assessment of functional abilities and needs, assisting clients in gaining access to needed financial and insurance benefits, referrals to medical and mental health services, employment and job readiness training, connection to social and education resources, life skills training, housing plan development and other supportive services and assessments. These support services include working with the client and their natural support system to develop and implement the service plan. It also includes follow-up to determine the status of services and the effectiveness of activities related to the successful implementation of the service plan to reduce/eliminate the need for institutionalization, hospitalization, incarceration and to enhance independence, autonomy, quality of life, housing stability, and inclusion in the community. Accurate documentation and billing of services is essential. Detailed, intricate, accurate data entry may also be required.

Duties and responsibilities

  • In tandem with the other Activities Case Manager, develop, plan, schedule, coordinate, and provide activities/social programs in compliance with the rules and regulations which govern ALF facilities in the state of Florida, which is a minimum of 12 hours per week, six days a week. This may include coordinating with other team members to provide activities/oversight.
  • Activities should include indoor and outdoor activities, onsite and offsite activities, shopping trips, creative, educational, exercise, and life skill development programs.
  • Provide transportation to offsite activities utilizing the facility vehicle.
  • Coordinate a monthly Resident Council Meeting where resident input for the facility is heard and pass along any grievances to the Administrator. Minutes of the meeting are to be submitted to the Administrator along with a sign in sheet for attendance.
  • Prepare a monthly calendar of activities in large print and post it in a prominent location that is visible to residents and visitors. Maintain record of these calendars.
  • Meet with each new resident to complete an assessment for areas of interest.
  • Encourage each resident to participate in activities and input a monthly note in the resident’s Electronic Health Record to document outcomes.
  • Recruit, train, and supervise volunteers as appropriate.
  • Coordinate with the kitchen staff for a monthly birthday celebration for resident’s whose birthday is within that month.
  • Direct Case Management Services:

Provides direct assistance to identified resident including completing assessments, person-centered service plan, and notes/documentation to facilitate the accomplishment of activities of daily living, personal service plan goals, encouraging community engagement, collaborating, and advocating with outside organizations. Demonstrates and coaches the development of skills identified as goals in the service plan. Assist residents in the removal of barriers to self-sufficiency, including applying for and maintaining benefits. Provide transportation for client to maintain medical or other appointments outside the program. Complete all assigned documentation and required notes including completing a progress note and upload required documentation into client’s Electronic Medical Record and other human services system if required by position within 24 hours of service. The frequency of resident contact will vary by need, with a minimum requirement of every two weeks. Responds appropriately and in compliance with agency Policies and Procedures to emergent incidents/accidents that develop with client and/or in the environment of care by contacting and reporting to the chain of command and 9-1-1 or the Abuse Hotline, if appropriate.

  • Supported Housing Services:

Orient and facilitate the resident with entry into RTF. Maintain safety, security, and satisfaction of identified resident by monitoring environment of care.

  • Must maintain appropriate boundaries in the workplace and work professionally (ethically), especially regarding confidentiality.
  • Check company email while on duty, responding timely, appropriately, and professionally, to communicate internally and externally.
  • Interact professionally with residents, co-workers, community partners, and vendors. Must be able to maintain tact and professionalism.
  • Other duties and responsibilities as assigned.
