POSITION SUMMARY:
Mentor will serve as a guide in order to assist client with navigating the community through their re-entry period. Services will be provided to adult clients
DUTIES AND RESPONSIBILITIES:
- Assist Case Managers in delivery of services
- Establish relationships with families of incarcerated adults prior to release
- Establish relationship with potential clients while incarcerated
- Assist in planning of activities for clients which includes but not limited to:
- Wrkshops
- Cmmunity Events
- Field Trips
- Creates and implements activities/workshops for clients and families.
- Provides a long term plan of action for each clients which includes direct services and referral services, including, neighborhood-based case management, relocation and transition, educational support, referrals to housing, mental health, substance abuse treatment, and tattoo removal etc.
- Advocates for the client when needed.
- Enroll clients in to job/school and other services needed to remain compliant with parole requirements
- Communicate with team on the daily basis regarding client progress
- Works with parents/guardians/siblings and provides services to the family, if needed.
- Coordinates all activities for the client and families with the various stakeholders in the person’s life including school officials, employers, probation officers, law enforcement officials, etc
- Monitors agencies to which clients are referred to assure quality service.
- Works collaboratively with other case managers.
- Coordinates and collates monthly program reports.

Any combination of experience and education likely to provide the required knowledge, skills and abilities, typically:
- 2-3 years of experience in mentoring and providing services to gang-involved population, including youth ages 14-18 years of age who are returning from juvenile camps.
KNOWLEDGE, SKILLS, AND ABILITIES:
Knowledge of:
- Principles and practices of organization and administration
- Principles of supervision, training and performance evaluation.
- Personnel policies and procedures.
- Use independent judgment and initiative.
- Applicable state and federal laws.
- Community resources.
- Human behavior and social science.
Skills and Abilities:
Ability to read, analyze and interpret general technical procedures, government regulations and laws.
- Ability to interpret a variety of instructions furnished in various forms.
- Ability to analyze and make competent decisions corresponding with the level of authority.
- Ability to operate standard office equipment, computer.
- Ability to follow SEA’s policies, procedures, and organization.
- Ability to meet attendance standards and work additional hours as necessary.
