The Douglas County Mental Health Initiative (DCMHI) Youth Care Compact Navigator is responsible for coordinating all aspects of The Youth Care Compact (YCC), a program of the Douglas County Mental Health Initiative (DCMHI). As fully designed and implemented, the Youth Care Compact (YCC) will build upon the DCMHI's Care Compact for adults, by offering a seamless, no-fail system for children, youth and their families with mental illness, substance use disorders, intellectual and developmental disability/delay (IDD) and basic needs through a care coordination network that addresses these conditions and associated needs.
Generally, the hiring range is $58,629-$73,2869 annually. Qualifications, education and experience as it relates to the position will be taken into consideration when determining hiring salary.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Assist in the research, development and implementation of the YCC pilot and full YCC program. Define processes, norms and responsibilities.
  • Collaborate with and leverage existing facilitated meeting structures for youth such as the Collaborative Management Program (CMP) and the Department of Health Care Policy and Financing's Creative Solutions to reduce duplication of effort.
  • Utilize Julota, the primary software system used by DCMHI programs to track client interactions, case manage, manage data and generate reports.
  • Act as the central coordinating entity of YCC by accepting referrals, determining eligibility, communicating with referral sources, creating and mobilizing care teams in alignment with a referral's level of urgency, and ensuring accountability and follow-through on action items. Execute this process within Julota to the extent possible, and utilize other means of communication to successfully carry out these activities.
  • Coordinate with The Care Compact (TCC) partners and other community partners to form care teams relevant to each client's needs.
  • Compile collateral and historical data to capture a holistic picture of the client's needs and goals.
  • Identify priorities with the child, youth and family at intake and continue to refine throughout YCC engagement.
  • Schedule initial and ongoing care team meetings to address child, youth and family priorities, and identify and assign a Lead for each case. If a Lead cannot be identified immediately or if there is a temporary vacancy of the Lead, the YCC Navigator may act in that role offering temporary case management.
  • Facilitate care team meetings, and act as a neutral party. With each care team, determine the frequency of meetings, follow-up on action items, prompt team updates and communication, and support case closure.
  • Manage and track signed Releases of Information (ROIs) for client cases, ROI expiration, requests to rescind a signed DCMHI Universal ROI (U-ROI), additions or changes to the signed ROI(s). The YCC Navigator will also collaborate with The Care Compact Navigator to refine the DCMHI Universal ROI (U-ROI)
  • Manage Care Team Member and Organization permissions in care coordination software, Julota.
  • Manage designation of Care Team duties to ensure balanced responsibilities, no duplication and clarification of roles. Assign Care Team members in Julota and determine the need to bring in additional partners as client's needs become clearer or evolve over time. Ensure reporting of services and emergency contacts are completed in Julota.
  • Evaluate the YCC program including establishing shared measurements of success among the YCC collaborative, ensuring data is collected from partner organizations and executing data sharing agreements where necessary, and regularly reporting data to the Board of County Commissioners, DCMHI Steering Committee and the DCMHI collaborative.
  • Continuously evolve the YCC program by expanding the partnership network and coordinating with other youth serving programs and organizations.
  • Work alongside first responders, mental health and substance use providers, IDD serving organizations, the single-entry point for long-term care and other supports, Department of Human Services staff, faith-based community partners, courts and the judicial system, and the Office of the County Attorney to support access to care, continuity of care and coordination for individual with behavioral health conditions.
  • When working with individuals and families, provide care and support that demonstrates cultural awareness and respect of their cultural beliefs, customs and norms.
  • Effectively meet and prioritize deadlines and multiple demands at any given time.

SUPERVISION RECEIVED: Position operates with a significant degree of independence receiving high level guidance and coordinating with the DC Mental Health Initiative Coordinator and the Deputy Manager. Position also works collaboratively with the Special Project Manager for Mental Health Programs, and closely with The Care Compact Navigator for the adults Care Compact program. Within the framework of the Youth Care Compact, makes high level decisions about the operation and facilitation of the Compact.
SUPERVISORY RESPONSIBILITIES: This position has no supervisory authority.
INDEPENDENT JUDGMENT: Work is performed with a moderate degree of independence, where work and decisions are regularly reviewed with a supervisor, but the employee is given latitude and individual judgment in selecting the proper means to achieve the desired result.
MINIMUM QUALIFICATIONS:
EDUCATION and/or EXPERIENCE:
  • A bachelor's degree in a relevant field (i.e. human behavioral health science, criminal justice, social work, etc.) is required. A Master's Degree in a relevant field (i.e., Social Work, Counseling, Psychology) is preferred.
  • Two years experience or more in care coordination, intensive case management or a related field is required.
  • Experience in clinical or cross-system care coordination or intensive case management is preferred.
  • Experience working with individuals with behavioral health needs, collaborative groups, and community professionals preferred.
  • A combination of education and experience may be considered.

KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge:
  • Knowledge of community resources and programming specific to youth, including but not limited to educational supports, state and local programming across multiple sectors (e.g., Child and Youth Mental Health Treatment Act - CYMHTA)
  • Program management and evaluation
  • Knowledge of, and ability to build competency in, federal confidentiality standards that impact the programs of the DCMHI including but not limited to the Health Insurance Portability and Accountability Act (HIPAA) and 42 CFR Part II.
  • Knowledge of the unique needs of youth; particularly within the behavioral health spectrum of care
  • Understanding of systems theory/philosophy, including community systems and family systems as they apply to whole person health determinants for youth.
  • Demonstrates framework for understanding social determinants of health and hierarchy of needs

Skills:

CERTIFICATIONS, LICENSES, & REGISTRATIONS:
  • This position requires successful completion of a criminal background check including fingerprinting through a national database.
  • Must possess a valid Colorado Driver's License upon hire, with complying insurance. Review of motor vehicle record is required at time of hire and periodically throughout employment.

WORK ENVIRONMENT:
Physical Work Environment: Hybrid work environment including office and home-based with field visitation and office-based meetings are typical. Travel to community partner locations is likely. Work scenarios involve complex and sensitive topics and information. May participate in ride-along trips with the CRT. Noise level is typically moderate. The position requires the employee to remain available via mobile cellular device/email access during normally scheduled business hours.
Physical Demands: The employee may occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision and distance vision. While performing the duties of this job, the employee is regularly required to talk and hear. The employee is frequently required to stand, walk, sit; use hands to find, handle, or feel and reach with hand and arms. The employee is occasionally required to stoop, kneel, crouch, or crawl.
Material and Equipment Directly Used: The position will require access to a computer, cell phone, and audiovisual equipment for presentations. While in the office the employee will have access to standard office and training equipment.
