Job description

General Overview:

The Youth Housing/Resource Case Manager plays a pivotal role in assisting Youth & Young Adults (YYA) in their journey to secure stable housing and access essential resources. This position is responsible for providing individualized support and connecting YYA with housing and community resources to enhance their independence and overall well-being.

Responsibilities and Duties:

Case Management

1. Receive cases and complete a screening to identify if the candidate will benefit from case management services.

2. Conduct motivational interviews with clients to determine their current resources and goals, as it pertains to helping them obtain housing, shelter, and/or employment.

3. Assist each client with creating a plan, prioritizing their needs by turning them into steps to help each client accomplish their goals.

4. Empower them to pursue their goals as it pertains to obtaining housing (or employment) and hold them accountable to those goals.

5. Provide support to recently housed YYA; promoting socialization, recovery, self-advocacy, and community living skills.

6. Participate in relevant CoC meetings and training sessions, upholding Brighter Days Guiding Principles.

Referrals

1. Keep and monitor a log of each organization in Central Florida that supports the sheltered and unsheltered. Update the log as needed on a monthly basis.

2. Refer clients to other organizations to obtain services and resources we do not offer, with a specific focus on housing & youth-oriented services.

3. Hold clients accountable to referrals and follow up with clients, systems, or organizations to confirm referral was followed through.

4. Utilize budget to fill service or resource gaps if there are no organizations that will meet a need that a particular client may currently have.

5. Actively recruit YYA to participate in the local Youth Action Board (YAB) and ensure their attendance in meetings.

Documentation

1. Keep detailed internal notes for each client in a secure location. Will ensure privacy and confidentiality of all notes taken.

2. Obtain signed Authorizations to Obtain and Disclose Information before entering info in HMIS if not already obtained.

3. Documentation will be done through the Homeless Management Information System (HMIS) to support the Continuum of Care (COC) coordinated effort to end homelessness. All referrals will also be documented in HMIS.

Service Support

1. At the direction of the Director, responsibilities may include assisting with outreach services.

2. Services include showers, laundry, distributing food, hygiene, clothing, facilitating haircuts, mail, storage, and our charging stations. As an example, this may involve helping to collect, wash and dry peoples laundry, while another station may be helping to clean and sanitize showers.

Transportation

1. Will drive to designated areas for street outreach at the direction of the Director.

2. Will use company vehicles for transportation to services when applicable.

3. May use company vehicles to purchase supplies or services for clients.

4. Will track mileage and be responsible to fill the vehicle up with gas.

Qualifications:

Requirements

● Must have a Bachelors in Social Work (BSW) or equivalent degree

  • Graduate level degree or certification preferred

● Excellent verbal and written communication skills

● Strong problem solving and interpersonal skills

● Organized accurate, thorough, detail-oriented and able to monitor work for quality

● Intermediate level with MS Office Suite and Google Docs

● Access to transportation with a current/valid driver’s license, a clean driving record, and proof of auto insurance, or demonstrated competency in the use of public transportation

Preferences

● Experience in working with the homeless population

● Experience in working with youth & Young Adults

● Graduate level degree or certification

● Bilingual (Spanish) highly preferred

Physical Requirements

● Ability to lift up to 50 pounds

● Ability to stand, walk, sprint, run and patrol for extended periods

● Must hear and see well (either naturally or with correction)

● Must be able to bend, stoop, squat, kneel, push, pull, and reach overhead

● A considerable amount of time making repetitive motions

● Work in a fast-paced environment with frequent interactions with customers

● Ability to function with frequent temperature changes between indoors and out

● Work in various weather conditions, including heat, cold, and rain

● Stand and walk continuously for up to 8-10 hours during a shift while providing customer care

● Considerable amount of time using a telephone and/or computer

● Sit at a desk and use a computer for extended periods to perform data entry and administrative tasks

● Answer phone calls and communicate with colleagues and clients verbally throughout the day

● Use hands and fingers for typing and operating office equipment, such as printers and scanners

● Use hands and fingers for precise mouse movements and keyboard shortcuts

● Review and analyze printed materials for accuracy and quality

● Attend meetings and collaborate with team members in various office settings

● Collaborate with colleagues and communicate professionally in person and via email

● Sit at a workstation and use a computer to respond to customer inquiries and resolve issues

● Use a headset to communicate with customers over the phone for extended periods

● Navigate between multiple software systems to access customer information

● Occasionally stand and walk within the office to consult with supervisors or assist colleagues

● Attend meetings and collaborate with team members when needed

Pay: $19.00 - $21.00 per hour
