VOA's Young Adult ShelterCase Manager position is a position that relies on independent and collaborative work to bring services and timely interventions to Young Adults ages 18-24.


We seek a workforce that reflects the diversity of the clients, whom we serve: people of many ages, racial and ethnic backgrounds, abilities, sexual orientations, gender identities, and religious beliefs.


The Young Adult Shelter (YAS) provides Young Adults ages 18-24 with a safe shelter 365 days a year. More importantly, the YAS offers life-saving and life-changing programs that provide medical care, food, and resources to the young people in our community who need it most.


The Young Adults who access the YAS most often have experienced or may be experiencing a variety of inequities and disparities; including but not limited to: homelessness, economic instability, high rates of chronic health and mental health conditions, traumatic events, lack of social support, etc. These clients, typically, have developed numerous skills that have helped them survive and meet their needs on the streets, while facing many barriers.


Position Type and Expected Hours of Work:

This is a full-time position with 40 work hours per week and set work hours. This position has a requirement of 8 work hours per day, Monday through Friday. Schedule may vary and may require long hours and varied shifts, as well evening, weekend, and holiday work. Additionally, this position requires being on-call on a rotating schedule.


Essential Duties and Responsibilities:

To perform this job successfully, an individual must be able to satisfactorily perform each of the essential duties/responsibilities listed below. The essential functions of the position include, but are not limited to the following:

  • Respect and value all clients, regardless of circumstance, behavior, or need.
  • Utilize Motivational Interviewing, Positive Youth Development, Harm Reduction, and Trauma Informed Care.
  • Work with Young Adults using creative strategies to develop trusting relationships.
  • Be knowledgeable of area resources for Young Adults and how to help them access those resources.
  • Build relationships with staff from systems of care for the purpose of sharing Young Adult referrals.
  • Assist Young Adults in getting their needs met through referrals.
  • Participate in community activities and events for the purpose of increasing awareness of homeless Young Adults needs and education regarding services and resources available.
  • Provide Case Management services to Young Adults including but not limited to:
    • Provide Case Management and facilitate access to emergency services whenever necessary, including medical, law enforcement, shelter and detox services for Young Adults, etc.
    • Create a client-driven case management plan with each Young Adult.
    • Assist Young Adults with paperwork to apply for services in the community.
    • Sit with Young Adults and help them schedule appointments and attend meetings to help them build trust with other service providers who can offer specialized services and case management.
    • Advocate for clients to help remove barriers to services in legal, educational and rehabilitation settings.
    • Assist clients in obtaining identification documents to remove barriers to housing.
  • Be able to work with a variety of personalities successfully and stay calm in tense situations.
  • Input accurate and complete data for all contacts with homeless Young Adults into the VOA agency database and the Homeless Community Information System (CMIS).
  • Meet program requirements for billable client services.
  • Comply with YAS and FCS case notes requirements and guidelines for documentation.
  • Understand, support and adhere to YAS policies and procedures.
  • Participate in required staff meetings including regular supervision
  • Complete daily outreach, referral logs and contact sheets as required.
  • Complete all required paperwork.
  • Ensure that client files and program records are maintained in accordance with agency, program and grant requirements.
  • Represent VOA at community meetings, agency networking and collaboration activities and events related to homeless Young Adults.
  • Understand, adhere to and support VOA agency policies, procedures and professional code of ethics and boundaries.

Additional Duties and Responsibilities:

  • Performs other related duties as required or assigned.
  • Employee must transport self in personal vehicle and may transport participants at Case Manager's discretion.
  • Travel is primarily local and during the business day, although very occasional out-of-the-area and overnight travel may be expected. 
  • This role has no supervisory responsibilities.

Requirements

To perform this job successfully, an individual must meet the minimum qualifications listed below. These qualifications are representative of the knowledge, skill and/or ability required to perform this job.


Required Education and/or Experience: To perform this job successfully, an individual must have: 

  • High School Diploma or GED/HSEC

Certificates and/or Licenses: To perform this job successfully, an individual must:

  • Be eligible to be licensed as Agency Affiliated Counselor (AAC) and apply for credential within 30 days of hire.
  • Become CPR/First Aid/BBP Certified within 6 months of employment
  • Obtain TB test or results of a negative TB test administered no earlier than 8 months prior to hire date
  • Have a valid Driver's License
  • Have the ability to maintain Auto Liability Insurance
  • Have reliable transportation
  • Successfully pass a Comprehensive Criminal History Background Check and Driving Record Review

Preferred Education and/or Experience:

  • Bachelor's Degree in a related field 
  • One (1) year of related experience working with youths in crisis who are low income, homeless, mentally ill and affected by substance abuse.
  • Two (2) years of experience working with at-risk, runaway, homeless/street youth

Bona Fide Occupational Qualifications:

Must be at least 21 years of age according to the program and funding requirements.

  • Knowledge of Medicaid and/or Medicare and/or Medicare enrollment services and process.
  • Proficient in word processing applications and associated software such as Excel, PowerPoint, Publisher, and Outlook; including Google Suite, CMIS (HMIS), and behavioral health software systems.
  • Proficient reporting skills including auditing data and processes regularly to produce reports and/or communications to applicable stakeholders.

Physical Requirements:

In order to meet all expectations of this position, an individual must be able to:

  • Regularly operates a computer with keyboard and mouse, as well as other office equipment, such as printers, copy machines, telephones, etc. Required to perceive text and buttons on the computer screen and discern print and handwriting on paper forms
  • Frequently required to remain in a stationary position (such as sitting or standing) and to move about inside and outside of the shelter to access file cabinets, office machinery, etc. Required to help lift and/or move client possessions, small to medium objects in the shelter, and/or cleaning supplies (up to 25 pounds).
  • Occasionally required to ascend and descend stairs, when accompanying Young Adults to various appointments. Required to help to lift and/or move food, shelter furniture, and large shelter items, such as furniture, large orders delivered to the shelter, etc. (up to 50 pounds).

While performing the essential duties/responsibilities of this job, the employee may experience the following environmental conditions:

  • Noise Conditions: The noise level in the work environment is often loud and chaotic consisting of entertainment activities such as music, games, and television, in addition to usual business office sounds including but not limited to computers, printers, and telephones.
  • Heat/Cold: May be subject to heat, cold, wet, and/or humid weather conditions, and occasionally extreme heat and/or cold.
  • Injury Exposure: position requires working with standard office equipment - copy machine, computers, and phones.
  • Atmospheric Exposure: Typical office environment conditions; may be exposed to various scents and perfumes, less than sanitary or uncomfortable conditions, various pests and pathogens, participants' service and emotional support animals, as well as clients in physical, mental or emotional distress.
