Position: Wound Care Specialist Nurse Practitioner - Homecare Services

Location: Greater New York Area

Job Type: Part-time, Traveling Position

Responsibilities:

• Provide wound focused provider services to patients in their homes, including assessments, diagnoses, and advanced wound treatments
• Develop personalized care plans in collaboration with patients, their families, and other healthcare professionals.
• Perform routine medical procedures such as routine Wound Care, debridements, wound vac application
• Educate patients and their families about management of their wounds between skilled services
• Coordinate with other members of the healthcare team to ensure continuity of care and optimal patient outcomes.
• Maintain accurate and up-to-date medical records and documentation.

Requirements:

• Valid Nurse Practitioner license in the state of New York.
• Master's degree in Nursing from an accredited institution.
• Certification as a Nurse Practitioner in the specialty area of practice (e.g., Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner).
• Minimum of 1 year of experience working in Wound Care
• Excellent clinical skills and ability to work independently.
• Strong interpersonal and communication skills.
• Willingness to travel to various locations within the Greater New York Area.
