Description
Whole Person Care provides comprehensive healthcare to people experiencing homelessness and other patients with complex medical and social needs. Whole Person Care serves thousands of vulnerable patients each year through case management, housing services, street outreach and street medicine, and the implementation of California Advancing and Innovating in Medi-Cal (CalAIM). The Whole Person Care program has been recognized state-wide for its innovative programs to meet the needs of people experiencing homelessness and has demonstrated a measurable impact on health outcomes for participants including reducing emergency room/hospital utilization, reducing readmissions, improving timely access to mental health and substance use treatment, and improving control of hypertension, COPD, and diabetes.
Under the supervision of the Chief Medical Quality Officer for Ambulatory care and in conjunction with the Director of Complex Care Management and Systems Integration, the Whole Person Care Physician lead would provide clinical guidance, conduct program development, and provide direct clinical care to support the Whole Person Care Program.
The ideal candidate is a physician who is passionate about providing high quality care for people experiencing homelessness and other patients with complex needs. This physician should thrive in a dynamic, interdisciplinary team, value health equity, and be interested in developing innovative services for Whole Person Care patients across settings. The Whole Person Care Physician champion will spend approximately 50% of their time providing direct patient care in a county primary care clinic and approximately 50% of their time doing programmatic Whole Person Care work, some of which will also be clinical.

  • Additional Pay - Low risk obstetrics on-call premium of $400 per weekday call or $600 per weekend call (UAPD MOU Sec. 502); Family Resident/Student supervision pay of $25 per day, up to $500 per month (UAPD MOU Sec. 504).

HEALTH CARE AGENCY AMBULATORY CARE- MEDICAL CLINICSThe County of Ventura Health Care Agency encompasses the Ambulatory Care department. Ambulatory Care is comprised of Federal Qualified Health Centers, such as primary care and specialty clinics. Thirty-Four clinics are located throughout Ventura County that provide health care services on an outpatient basis. Outpatient health care services range from routine care to chronic medical conditions.
The goal of the Ambulatory Care is to allow healthy people to thrive in healthy communities throughout Ventura County. This is achieved by a team of dedicated physicians, nurses and other health care professionals that seek to set the standard in health care excellence.
The Primary Care Clinic Physician is an At-Will classification.
Examples Of Duties
Duties may include but are not limited to the following:
  • Provides clinical supervision of the Whole Person Care case management team, including supervising weekly case conferences, developing and signing off on care plans, and providing in the moment clinical guidance to WPC teams.
  • Assists in development of new programs, clinical workflows and service lines to meet the needs of people experiencing homelessness as well as the other populations of focus in Cal-AIM.
  • Provides medical care on the street, in homeless encampments, and in other non-traditional locations, including care for substance use disorders.
  • Offers guidance on how to shape clinical workflows to meet reporting/regulatory requirements and patient needs
  • Provides mentoring and oversight to Whole Person Care Physician champions and care management teams located at community clinics throughout the system
  • Provides complete, comprehensive, family-oriented, culturally-sensitive clinical care for patients throughout the lifespan (or to adults for internal medicine physicians), including obtaining medical histories, performing physical examinations, making diagnoses, and treating patients.
  • Prescribes medications and treatment, monitors patients' progress, and adapts treatment as required.
  • Inoculates and vaccinates patients to immunize patients from communicable diseases.
  • Advises patients concerning evidence-based lifestyle changes that improve patient health and works with patients to develop self-management goals.
  • Coordinates with care team to provide high-quality patient care and ensure patient treatment goals are met.
  • Works with a high volume of patients, internal/external customers, and must appropriately manage frequent changes, delay, or unexpected events.
  • Follows workflows that support quality improvement efforts and increases patient access.
  • Explains in clear understandable verbal and written language the care plan and any procedures.
  • Advises nursing and related personnel as to the administration of medication and treatment; writes physician's orders.
  • Gives or determines the need for medical tests, orders laboratory tests as necessary; interprets medical tests.
  • Consults with other physicians and providers, refers patients to specialists when necessary, and works with other healthcare staff, such as nurses, assistants, and other physicians.
  • Keeps accurate and timely medical records and charts and adheres to documentation policies. Prepares various records such as progress notes, comprehensive care plans, summaries, etc. Documents gathered information in medical record as required by policies.
  • Keeps updated problem lists and medication lists for all patients.
  • Responds to portal messages and EHR inbox in a timely fashion and provides coverage for colleagues' inboxes when they are not available.
  • Counsels patients and families about physical, medical, and mental conditions; obtains appropriate consent for treatment from patients or parent/caregiver;
  • May participate in an on-call system.
  • Participates in meetings, conferences, and training sessions as determined by Ambulatory Care administration.
  • Follows safety protocols, including but not limited to using two patient identifiers and time-out procedures.
  • Keeps all medical staff privileging requirements up to date, including CME requirements.
  • Administers first aid or cardiopulmonary resuscitation in an emergency.
  • Complies with medical laws (for example, HIPAA). Demonstrates knowledge of domestic violence, child and dependent abuse protocols and mandated reporting.
  • Performs other related duties as assigned.

Typical Qualifications

EDUCATION/EXPERIENCE
  • Completion of an ACGME-accredited residency program
  • Degree in Medicine (either Doctor of medicine or Doctor of Osteopathy)
  • At least 3 years of residency training
  • Current state medical license and state DEA certificate
NECESSARY SPECIAL REQUIREMENTS: Must be able to meet the VCMC Medical Staff Credentialing requirements as outlined in the
  • Clinical experience caring for persons experiencing homelessness, with serious mental illnesses and/or substance use disorders, ideally in a county delivery system or FQHC
  • Experience designing, implementing and/or overseeing programs to address the comprehensive social and medical needs of individuals with complex needs.
  • Experience implementing quality improvement initiatives, health system transformation initiatives, and/or Medi-Cal programs
  • Board certification in family medicine or internal medicine

HIGHLY DESIRED:
  • Master's degree in business administration, public administration, public health, or a related field
  • Experience providing clinical supervision to interdisciplinary case management teams
  • Board certification in addiction medicine or geriatrics
  • Experience leading street medicine teams, providing clinical care in permanent supportive housing or recuperative care.
KNOWLEDGE, SKILLS, and ABILITIES:Knowledge of: current developments, trends, best practices, and evidence-based guidelines in the medical field; basic understanding of clinical supervisory principles and practices; pharmacology and psychopharmacology including laws regarding informed consent, dosage ranges, drug incompatibilities, and side effects; proper documentation of clinical services; principles, practices, and techniques of general medicine and its application; medical diagnosis and treatment; hospitals and clinics organization, communicable disease control; public health practices and procedures.
Skill/Ability to: diagnose and treat patients; develop and monitor appropriate treatment plans; establish and maintain effective working relationships with patients, their families, staff and others; learn and apply laws, statutes, codes and regulations governing California health service providers, and Joint Commission and other regulatory requirements; maintain medical records and prepare reports; serve on interdisciplinary teams; clinically supervise licensed and professional staff.
