JOB SUMMARY
The Supervisor Care Coordination supervises the daily activities and staff accountable for working directly with members/patients who have high needs and/or high costs due to chronic health issues, have gaps in care, are difficult to engage or any other people deemed appropriate. Works closely with teams across many departments, provider offices and community resources to foster timely delivery of quality, accessible health care services.
ESSENTIAL FUNCTIONS OF THE ROLE
- Provides technical and administrative help and supervision of staff under industry best practices, including hiring, performance management, evaluations, education, staff development, work allocation and problem resolution.
- Provides oversight to staff in guiding members/patients through the healthcare system and works to overcome non-clinical obstacles that prevent them from receiving the care and treatment they require. Trains and helps staff in identifying needs of the people and families served to ensure quality of care.
- Collects and evaluates data related to gaps in care and outcomes; develops new or enhances existing processes to outreach effectively and efficiently to engage members/patients, improve health outcomes and reduce health care disparities.
- Participates in development, coordination, delivery and quality of services oversight for non-clinical staff.
- Identifies and implements standards of practice and care to achieve optimal clinical, financial and member/patient satisfaction outcomes.
- Helps clinical heads with developing relationships and processes to facilitate participation by physicians, pharmacists, nurses, social workers and other health care professionals in multidisciplinary care planning, care delivery and discharge planning.
- Participates in the development and implementation of standardized policies to achieve efficiency of operations and the provision of optimum care.
- Ensures compliance with all institutional policies and regulatory requirements.
- Oversees the development of discharge plans and strategies that address needs through the continuum of care post-discharge.
- Ensures appropriate scope of practice by staff. Ensures that subordinates are familiar with prominent ethical standards in the profession, related to issues such as confidentiality, informed consent, conflicts of interest, dual relationships, boundary issues, documentation, consultation, referral, and end of services.

- Knowledge of human behavior and performance; separate differences in ability, personality and interests; and psychosocial methods.
- Knowledge of the standards and methods to restore or enhance social, psychosocial or bio-psychosocial functioning of people, couples, families, groups, organizations and communities.
- Ability to help clinical heads develop structures, relationships and processes to deploy multidisciplinary care planning, care delivery and discharge planning.
- Ability to standardize policies, procedures and programs.
- Relational skills to interact with a wide range of constituencies.
- Verbal and written communication skills.
- Ability to make tactical selections around the allocation and direction of the work of supervised employees using established guidelines, policies and procedures.
- Ability to make effective hiring and performance management recommendations.
- Ability to train and evaluate performance of supervised staff.
- Skill in the use of computers and related software.
- Community Health Worker (CHW) certification preferred.
- Four (4) years of related experience required; two (2) years of supervisor experience preferred. Supervisor experience may be included in the total four (4) years of related experience.
