The Wellness Recovery Navigator is responsible for assisting program participants in accessing eligible benefits and entitlements, including social security, SNAP, Medicaid. The Wellness Recovery Navigator provides individual and family support; outreach; behavioral health and, substance use disorder services refenals and warm handoffs; referral to housing and employment resources; and suppo11 for social-leisure, and other activities. The Wellness Recovery Navigator is responsible for enrolling pat1icipants in Wellness and Rec.:overy Access Program C2C (WRAP C2C), including the completion of the GPRA, behavioral health and substance use disorder screens, and detem1ining eligibility for additional services and benefits. Tbe Wellness Recovery Navigator will follow-up und track prograrn participants' engagement in the WRAP C2C using the approved management information system and provide data to Program Coordinator consistent with data reporting requirements. The Wellness Recovery Navigator will be part of the team that will staff and prnmote the Wellness and Recovery Access Program C2C (WRAP C2C).

Duties of the position:

• Promote recovery through modeling own personal self-confidence, achievements, and self-determination
• Welcome and engage program participants into WRAP C2C
• Provide mutual support, hope, reassurance, and advocacy
• Help clients aspire to roles that emphasize their strengths by sharing their personal experiences / stories
• Provide peer support to clients regarding their symptoms and the effects of trauma and trauma history, and helps them to develop positive coping skills and use of natural supports
• Engage clients through outreach and support
• Provide training and education to other team members on recovery-oriented care, peer support resources, psychiatric advance directives, wellness tools, trauma-informed care, and the use of creativity and the arts to promote recovery
• Attend interdisciplinary team meetings
• Assist person served to complete peer-related elements of the assessment and to prepare to attend their recovery plan meeting and to express their goals and needs
• Assist clients to accomplish their life goals of living in their chosen housing and community, working, having a natural support system, and having hobbies and other leisure activities which are creative and fun.
• Help clients and staff to develop and implement person-centered treatment plans.
• Facilitate peer recovery support groups.
• Act as an advocate for clients to obtain needed services and public benefits and to raise complaints and suggestions about unmet needs
• Provide on-call crisis intervention services as scheduled Attends the treatment team of all individuals for whom s/he is providing service and any individual that has been interested in receiving peer support

Required Education and Qualifications:

• The Wellness Recove1y Navigator will have a bachelor's degree in psychology, counseling, human services, social work, nursing, rehabilitation or related field from an accredited college or university OR meet the criteria of an experienced Paraprofessional in having a High School Diploma and 5 years of experience in the field. The Wellness Recovery Navigator will have a minimum of 5 year of' experience working with persons with a severe mental illness and/or substance abuse. At least one year’s experience working in health services
• Experience working with underserved populations preferred
• Excellent communication skills and organizational skills
• Excellent ability to work with clients with substance abuse, co-occurring and/or mental health disorders
• Certification as Peer Recovery Specialist preferred but will consider a candidate with lived experience and a desire to pursue certification
• Must be available to work weekends.
• CPR and First Aide Certification
• Naloxone Administration Training
• MANDT Training
• Training in Trauma Informed Cure
• Training in Infection Control
• Training in WRAP and/or other tools that promote illness Management and Recovery
• Training in PM-46-identification and reporting of abuse, neglect) mistreatment, and misappropriation or property/financial exploitation
• Valid Drivers License in good standing with at least three years minimum driving experience with no serious traffic infractions
• Training in 45 and 42 CFR, Rules and Regulations that safeguard a person's privacy (HIPPA)
• Training about local resources in the community

Additional Qualifications:

• Computer knowledge of MS Office, and other HR applications is a plus, not required.
• Possess strong verbal and written communication skills.
• Must be proactive, organized, and possess great follow-up skills.
• Must have the skill/ability to collaborate, partner with, and provide exceptional customer support to a variety of affiliate partners within the organization.
