General Responsibilities and Accountability (Registered Nurse)
Maintain appropriate confidentiality and discretion at all times.
- Affirm the company’s vision and mission statements and uphold the core values caring, integrity stewardship and excellence.
- Infuse all aspects of the position with companies’ holistic wellness philosophy - “Collaborative Care for a Healthier Home”.
- Perform other related duties as needed or assigned.
- Demonstrate daily work habits which show commitment to and compliance with the leadership expectations of company and the values and expectations statements related to our compliance program.
- Coordinate training and team meetings to assure all Team Members are properly trained on clinical procedures, companies’ policies and procedures, and other topics.
- Assess quality of care provided by the Wellness Program.
- Attend Risk (Care Transitions) Meetings.
- Maintain role as a liaison between Home Care, Governing Bodies, Group of Professional Personnel, Staff Members and Community.
- Provide up to 15 contracted Home Health visits.
- Provide support for Home Care when needed.
- Provide Wellness Hours for Blood Pressure Checks, Triage Call, One-On-Ones.
- Work with Intake to onboard new referrals for Home Care or Health Care.
- · Meet at least weekly with onsite social work team and attend meetings to go over care.
- Assure Staff Members are meeting policy.
- Participate in budget planning, and overall compliance, including billing for service.
- Prepare required reports for company.
- Attend committee meetings as assigned.
- Oversight of Team Member scheduling and supervision of team members.
- Meet with Home Care and Home Health Staff.
- Follows policies and procedures of Continuous Home Care and secure assistance from Supervisor/Management when needed.
- Provide management with required licenses and other documents as per JCAHO and the state of PA.
- Be oriented to EMR (Electronic Medical Record, KINNSER) especially upon data entry, timely document completion and HIPPA patient confidentiality.
Education and Licensure
· LPN, Registered Nurse
· Current PA LPN / RN license as applicable
· Health care Supervisory experience
· Current Driver’s License, use of vehicle and auto insurance
· Meet requirements of employment at company
· Current TB testing and/or chest x-ray.
Competency and Skill Requirements
· Strong communication skills.
· Ability to read, write and understand English.
· Strong administrative and computer skills.
· Good Leadership skills.
· Ability to function independently.
· Knowledge of Sales and Requirements for Home Health and Home Care.
· Compassion and understanding for all Residents and Clients.
