The Wellness Advocate fulfills the job requirements of the Care Manager Extender in the NC Tailored Care Management guidelines.
HealthKeeperz is actively seeking dedicated Wellness Advocates to support our Tailored Care Management program in Fayetteville, NC. This vital role supports comprehensive care management across multiple domains, including physical health, behavioral health, I/DD, TBI, pharmacy, LTSS, and addressing unmet health-related needs. Enhance our team's capacity by enabling care managers and supervisors to focus on members with intensive or complex needs.
Requirements:Key Responsibilities:
- Conduct outreach, engagement, and follow-up with members.
- Coordinate services and appointments, including reminders and arranging transportation.
- Participate in health promotion activities and share vital information with the care team.
- Assist with tracking referrals, accessing community resources, and social support services.
- Support care managers in assessing unmet health-related needs.
- Maintain regular communication with Care Managers and Supervisors to streamline member outreach.
Skills and Qualifications:
- Proficient in person-centered planning and motivational interviewing.
- Strong knowledge of Medicaid and waiver benefits plans, and LOC processes.
- Excellent interpersonal, communication, conflict resolution, and organizational skills.
- Proficient in Microsoft Office and able to make independent decisions.
- High school diploma or equivalent, with specific training in Tailored Care Management.
- Experience or lived experience in navigating the NC Medicaid delivery system, with roles open to Certified Peer Support Specialists, Community Health Workers, or those with relevant personal or professional experience.
