A Successful Nurse Liaison has the following:

Responsibilities:
- Utilize expert-level assessment skills and critical thinking to implement on-site referral coordination for all TL service lines.
- Coordinate with appropriate hospital staff to promote a seamless transition from hospital discharge back to the community setting, or hospice inpatient setting, with TL support.
- Work collaboratively with the IDG Team and other internal/external partners in achieving patient care goals
- Use professional nursing judgement to individualize the plan of care based on assessment of the unique needs of each patient.
- Maintain comfort, privacy, and dignity of our patients
- Develop working knowledge of all federal, state and company regulations, policies, and procedures; ensure compliance
Requirements:
- Registered Nurse with current NC license; BSN preferred.
- Must have reliable transportation
- Must submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal guidelines
