We are seeking a compassionate and skilled Registered Nurse to join our team. As a home health/hospice RN, you will play a crucial role in delivering essential medical care to patients in their homes. Your dedication to providing high-quality, compassionate care and meticulous documentation in the Electronic Medical Records (EMR) system will significantly impact our patients' well-being and recovery.

Responsibilities:

  1. Patient Care and Assessment:

    • Perform thorough patient assessments to create and execute individualized care plans.
    • Administer medications, wound care, and specialized treatments as prescribed.
  2. Monitoring and Education:

    • Monitor patient conditions, offer education, and support patients and families in managing illnesses and recovery.
    • Provide guidance on healthy practices, medication management, and disease prevention.
  3. Collaboration and Documentation:

    • Coordinate care with the interdisciplinary team, ensuring holistic patient well-being, and meticulously document patient progress and care provided in the EMR system.
    • Maintain accurate and timely documentation to ensure comprehensive patient records.
  4. Emotional Support:

    • Provide emotional support and empathy during challenging periods of illness and recovery.
    • Advocate for patients' needs and preferences, ensuring their well-being and comfort.
  5. Adaptability and Problem-Solving:

    • Adjust care plans as needed, applying critical thinking and problem-solving abilities in varied home settings.
    • Address unexpected situations or emergencies effectively and efficiently.

Role Requirements:

  1. Education: Registered Nurse (RN) license in good standing and completion of a nursing program from an accredited institution.

  2. Experience: Prior experience in home health or a related field is preferred.

  3. Compassion: Demonstrated empathy, sensitivity, and a patient-centered approach to care.

  4. Communication Skills: Excellent verbal and written communication for effective patient and family interaction.

  5. Adaptability: Ability to work independently and adapt to different home environments while ensuring patient safety.

  6. Ethical Conduct: High ethical standards, maintaining patient confidentiality, and respecting their autonomy.

  7. Technical Skills: Proficiency in utilizing Electronic Medical Records (EMR) and other healthcare software.

  8. Driver's License: Some roles may require a valid driver's license and reliable transportation for home visits.
