RN Case Managers at the VNH screen, evaluate, re-evaluate,and treat patients as well as coordinate and communicate treatment activitieswith our interdisciplinary team members and the patient.
The RN Case Manager:
- Develops a care plan in collaboration with the patient'sphysician, the patient, and their family, or the primary caregivers uponadmission. Periodically reviews the patient care plan and recertifies thepatient care plan as appropriate.
- Coordinate the patient's care plan through interdisciplinarycommunication and case conferences, as appropriate. Advise other disciplines ofdischarges in a timely manner.
- Provide ongoing assessment of the patient's clinical,functional, and emotional status to identify problems and/or progress towarddesired goals. Communicate significant changes to the patient's physician andVNA colleagues.
- Document evaluations, treatment goals, and objectives.
- Teach the patient and caregiver self-care techniques asappropriate. Provide medication, diet, and other instructions as ordered by thephysician and utilizes opportunities for health counseling with patients andcaregivers.
Minimum Qualifications:
- Must be a licensed Registered Nurse in Vermont and NewHampshire or hold a compact license.
- Graduate of an accredited school of nursing.
- Medical/Surgical nursing and/or home health care experience.
