Job Title: Veteran Outreach Coordinator

Overview:
As a Veteran Outreach Coordinator for our senior caregiving agency, you will play a crucial role in identifying, engaging, and supporting veterans in need of caregiving services. This position requires a deep understanding of the unique challenges and needs faced by veterans, particularly those in their senior years. The Veteran Outreach Coordinator will work closely with veterans, their families, and community organizations to ensure our senior caregiving services meet the specific requirements of this honorable demographic.

Responsibilities:

Identification and Outreach:

  • Conduct outreach activities to identify veterans who may benefit from our senior caregiving services.
  • Establish and maintain relationships with local veteran organizations, VA offices, and other relevant community groups.
  • Utilize various channels, including social media, community events, and partnerships, to reach and engage veterans.

Assessment and Needs Analysis:

  • Conduct comprehensive assessments to understand the specific needs and challenges faced by veteran clients.
  • Collaborate with healthcare professionals and agencies to gather relevant medical information and create personalized care plans.
  • Education and Advocacy:
  • Educate veterans and their families about available senior caregiving services and benefits.
  • Serve as an advocate for veterans, assisting them in navigating the complexities of healthcare systems and accessing appropriate resources.

Collaboration with Internal Teams:

  • Work closely with the caregiving team to ensure that services are tailored to meet the unique needs of veteran clients.
  • Collaborate with case managers, social workers, and other professionals to provide holistic support.

Community Engagement:

  • Organize and participate in community events to raise awareness about senior caregiving services for veterans.
  • Develop and maintain partnerships with local businesses, government agencies, and nonprofits to enhance the support network for veterans.

Documentation and Reporting:

  • Maintain accurate and up-to-date records of veteran clients, including assessments, care plans, and service utilization.
  • Prepare regular reports on outreach efforts, successes, and areas for improvement.

Qualifications:

  • Previous experience working with veterans or in a related outreach role is preferred.
  • Knowledge of veteran benefits, healthcare systems, and community resources.
  • Strong communication and interpersonal skills.
  • Empathy and understanding of the unique challenges faced by senior veterans.
  • Ability to work independently and as part of a collaborative team.
  • Organizational and time management skills.
  • Proficiency in basic computer applications.

Education:

  • Bachelor's degree in a relevant field (Social Work, Public Health, Psychology, etc.) is preferred.

Job Type: Full-time

Pay: $45,000.00 - $55,000.00 per year
