Job Summary:

TriStar Centennial Medical Center’s premier cardiovascular consultant group is seeking a certified Acute Care provider for a VAD coordinator role in Nashville, Tennessee.
Qualified Candidates:
 

• Acute Care Nurse Practitioner or physician assistant certification
• Tennessee licensure or ability to obtain
• Two years hospital experience in a cardiac or CV surgical ICU
• Experience caring for mechanical circulatory support and advanced heart failure patients is preferred
• Full time, 4 10-hour shifts
• Shared call 1:3 with other coordinators

Job Responsibilities: 
 

• Coordinates and participates in patient evaluation and selection for advanced therapies
• Coordinates/leads patient selection and performance improvement meetings
• Outpatient clinic visits, clinical management, and care coordination (shared call with 2 other coordinators)
• Inpatient discharge planning/care coordination 
• Inpatient and outpatient education to VAD patients and their caregivers
• Oversees/provides education to Acute Care staff and community resources (EMS, rehab, home health, etc.)
• Participates in the development and maintenance of VAD statistical databases and associated registries (INTERMACS) 
• Equipment and supply management using online inventory tool 
• Joint Commission certification preparedness
• Involvement with community outreach on VAD safety measures and awareness
• Liaison between surgeons, heart failure cardiologists, industry partners and patients
