Dates of Coverage:
- Aug 23 (6am) - Aug 26 (6am)
- Sept 20 (6am) - Sept 23 (6am)
- Oct 11 (6am) - Oct 14 (6am)
- Nov 1 (6am) - Nov 4 (6am)
- Nov 28 (6am) - Dec 2 (6am)
- Dec 20 (6am) - Dec 23 (6am)
- Reason for Coverage: 1 Physician is leaving to join the Mayo Clinic in Arizona/ another physician is cutting back on call coverage
- Call Schedule: 24-hour call for dates listed
- 100% Adult/ Geriatric
- ED & Inpatient Only
- Average Number of Office/Clinic Patients per Day: NA
- Average Number of Hospital-Based Surgeries per Day:): 2-5 per weekend
- Other Urologists will not be Present During Locum's Assignment
- Urologists in the group will be 4.
- Lab and X-Ray Onsite
- Describe Office Ancillary Staff (e.g., Number of RNs, LPNs, MAs, PTs, OTs, etc.): NA
- Describe Office Equipment (Specialized Equipment, etc.): NA
- Average Number of Patients Per Day: 15-20
- Documentation System/EMR: Epic
- Trauma Level/Designation: II
- Board Certified
- Active SC license
- 100% CLEAN BACKGROUND - HIGHLY Preferred; may consider small malpractice from years ago depending on situation
- No new grads, must have great experience, ratings, and training
- BLS/ACLS - preferred
