· Department description: Urgent care clinic, outpatient. Provider will be responsible for delivery of care, treatment, and referrals. Perform minor clinical procedures for patient’s benefit within scope and skill of practitioner.

· Board Certification: Yes, Urgent Care
· State license required: active OR license
· DEA: yes
· Minimum experience required: 1 year Urgent Care
Credentialing Timeframe: 45-60 days
