What you bring to SBHG:
- High school diploma or equivalent required.
- (1) year unit clerk or clerical experience and medical record experience in an inpatient or outpatient setting.
- Valid California Driver's License, required.
How you will make a difference:
- Maintains and creates client binders on the unit in appropriate chart order, and in a neat, clean and orderly condition.
- Assures that client binders will be properly labeled with clients names and other required information (e.g. allergy stickers)
- Informs medical and clinical personnel of required screenings, assessments, and documentation needed per regulations and tracks completion accordingly
- Photographs new admissions and insures that one photo each will be placed in the medical record and in the medication book.
- Completes daily census and sends to appropriate county and agency personnel
- Updates Census and Shift Report and posts electronically and within the O Drive
- Supports intake and referral process
- Updating/ maintaining/ sending County Access log- referrals: intakes & declined
- Scans documents into EMR, audits and ensures completion and accuracy of documentation
- Orders necessary nursing, clinical and clerical supplies for the unit as determined by the Administrator and/ or DON.
- Schedules lab work, EKG, EEG, X-ray, Podiatry, Dental and other consultant appointments, history and physicals, physical, speech and other therapies and other ancillary services ordered by the physician of the treatment team, and completes required forms.
- Coordinates appointments for Charge Nurse taking into consideration staff requirements,
- space availability and third party payor reimbursement.
- Answers telephone and takes messages at assigned unit.
- Prepares transfer documents for clients, photocopying relevant information from the chart per facility policy.
- Visually checks medical records daily for unsigned physician orders and attached red "flags" to those orders. Notifies Medical Director of any physician order not signed within the required 5-day limit.
- Updates the room assignment board electronically.
- Visually monitors lab supplies or is notified of supplies needed and orders supplies by telephone or in writing from the lab vendor.
- Keeps communication book adequately supplied with forms.
- Monitors medical record forms supply twice weekly and obtains forms from Medical Records Department on a regular basis or as needed.
- Obtains telephone or written pre-approval and documents same for medical treatment or testing for third party providers as required.
- Enters client information into county MIS.
Division/Program Overview:
- 24/7 inpatient involuntary program
- 16-bed facility for acute youth (ages 12-17)
- Intensive treatment programs with individualized care plans
- Available for contracting Central Valley counties
- Moves clients through five phases: admission; engagement; learning; stabilization and linkage to resources and aftercare
