POSITION SUMMARY:
The purpose of this position is to provide quality, comprehensive nursing services to Tribal individuals and families. This role focuses on the prevention of illness and promotion and maintenance of health. The Tribal Member Well-Being Nurse shall provide therapeutic services, home visits, education, and advocacy services. This is accomplished through the assessment and identification of the individual, family, and community needs, establishing health goals, and planning of programs that meet identified needs and coordinating health programs and services. The Tribal Member Well-Being Nurse is also responsible for the supervision of the Tribal Member Well-Being Department in a manner that is consistent with the philosophy and objectives of TMWIHC. This individual will ensure that systems are in place to safeguard patients medical information in accordance with HIPAA regulations and requirements.
PRIMARY RESPONSIBILITIES: (will include, but not be limited to)
1. Administration and Planning
A. Annually, review and/or revise Tribal Member Well-Being Department policies, procedures, objectives, and work plans.
B. In conjunction with the Medical Director or his/her designee, develop short- and long-range plans for improving community health standards. Such plans will incorporate evaluating tools designed to assess current community health standards.
C. Develop and implement staff in-service training on a regularly scheduled basis.
D. Develop and submit proposals for funding of Tribal Member Well-Being Department objectives.
E. Coordinate the collection and tabulation of data, which represents all monthly activities of staff.
F. Establish and maintain a tracking system to assure timely and appropriate action on referrals to and from the Tribal Member Well-Being department.
2. Staff Supervision and Evaluation
A. Provide orientation to all new Tribal Member Well-Being staff.
B. Supervise and evaluate the performance of department staff
3. Be familiar with the aims and objectives of the Health and Well-Being for our Tribal community and express a genuine enthusiasm for success.
4. Regular and consistent attendance is an essential function of this position.
5. The ability to appropriately and professionally interact with and to get along well with others is an essential function of this position.
6. Maintain the Diabetic Program.
7. Other duties as assigned.
QualificationsQUALIFICATIONS:
1. Valid California Licensed Vocational Nurse license.
2. Current BLS certification.
3. Valid California Driver's License.
4. Five years experience as an LVN in a clinic or office setting.
5. Must demonstrate effective written and oral communication skills.
6. Experience with and the ability to operate Microsoft Office software is required. Experience working with the RPMS computer system is preferred. Must possess skill in using computers and other office equipment; in composing correspondence; managing filing systems; gathering and summarizing information; and in planning, organizing, prioritizing, and executing complicated and continuing referral assignments. Must have the ability to correctly enter narratives and data in RPMS and the Electronic Health Records system.
7. Must have the ability to work in an independent and self-directed manner and assume individual initiative. Must be willing and have the ability to accept supervision and to follow directions.
PHYSICAL REQUIREMENTS:
Bending, squatting, stretching, and lifting up to 50 pounds on a regular basis. On occasion, may be required to lift items weighing 50 to 100 pounds. This position requires walking, standing and sitting for prolonged periods of time; climbing step stools, utilizing 20-foot ladders, stooping, kneeling, crouching and reaching; pushing, pulling and lifting up to 100 pounds; raising objects overhead; manual dexterity for operation of computer, phones and small office machines; hearing, talking and good verbal and written communication skills; repetitive motions of the wrists, hands, and fingers. This position requires the ability to safely operate a motor vehicle.
