This is a Remote position for a Registered Nurse/RN living in Michigan. We specialize in providing Home Health and Hospice in the home and facilities. KEY JOB RESPONSIBILITIES: Responsible for triaging and resolving issues for Hospice. Maintaining the Home Care/Home Base database for accuracy. Responding to the needs of the patients, families and physicians calling into Triage with any issues. Schedule field staff in an accurate and timely manner.

Triage Registered Nurse/RN Responsibilities:

  • Triage patients requiring urgent and emergent medical advice evenings, weekends and holidays via telephone.
  • Triage Registered Nurse/RNprocess incoming hospice referrals
  • Triage Registered Nurse/RN assign Admission, evaluation, subsequent, and discharge visits within EMR.
  • Triage Registered Nurse/RN reassign Admission, evaluation, subsequent, and discharge visits within EMR.
  • Triage Registered Nurse/RN run scheduling reports.
  • Triage Registered Nurse/RN assist field staff with ordering durable medical equipment (DME) and patient supplies as needed.
  • Triage Registered Nurse/RN assist with start of care admission process, displaying proficiency in the identification of hospice eligibility requirements set forth by Medicare guidelines.
  • Triage Registered Nurse/RN educate both the patient and family on medication management, treatment protocols and end of life processes along with identifying comprehensive and safe understanding of caregiver ability to provide safe care.
  • Triage Registered Nurse/RN engage in effective interdisciplinary communication with various professions of the hospice care team to promote the most effective care for the patient and their family.
  • Triage Registered Nurse/RN proficiently document all health assessments, physician orders and communication with the patient, their family and various medical professionals in a timely manner.
  • Triage Registered Nurse/RN maintains patient confidentiality, including all HIPAA regulations.
  • Triage Registered Nurse/RN compliance with agency policies and procedures regarding operations/processes, including but not limited to those regarding patient care, patient complaints, incidents, safety and emergency management.
  • Triage Registered Nurse/RN manages Territory with minimal supervision based on forecasted needs, priorities and business strategy.

 

Qualifications

PROFESSIONAL EXPERIENCE/QUALIFICATIONS:

  • 1+ year of previous Hospice nursing experience.
  • Minimum of one year of experience in medical/surgical and one year of experience in critical or emergency care with specialized triage training.
  • Strong written and verbal communication skills.
  • Excellent customer service skills.
  • Commitment to The Care Teams philosophy of care, values, vision and mission statements as an exemplary role model for all staff.
  • Must have at least one-year experience in the Hospice industry.
  • Knowledge of Home Health is preferred.

Skills

  • Must be able to make good professional decisions quickly despite stressful atmosphere.
  • Ability to handle stressful and constantly changing conditions.

Regulatory Requirements

  • Must live in the state of Michigan
  • Graduate of an accredited school of nursing, with a bachelors degree preferred.
  • Current state license as a registered nurse and willing to become licensed in other states
  • Must pass a criminal background check & MVR check.
  • Completed health statement acknowledging ability to perform the duties of the position.
  • Valid state drivers license.
  • Proof of current automobile insurance.
  • TB testing per agency policy; (1 or 2 step TB skin test within 12 months of hire & annual TB symptom screening thereafter).
