AuthoraCare Collective is currently seeking a TriageRN After Hours position (Home Care/Long Term Care) for our Greensboro service area. This position is Full-time (32 hours) Friday 4:30pm - 12:00am; Saturday and Sunday 7:30am - 8:30pm.**Training must be completed during the work week between the hours of 8am-5pm/flexible on days**
Job Summary:
Assess and identify patient/caregiver problems, establish patient/caregiver care goals, and identify and implement interventions for all agency programs. Work with attending physician to establish and implement medical plan of care for the patient. Work closely with other members of the interdisciplinary team and communicate effectively both verbally and in writing with team members, patients, caregivers, physicians, pharmacists, and other health care professionals and community service organizations.
Education and Experience:
- Must be currently licensed as an RN in North Carolina/ Graduate of an accredited School of Nursing
- One year of Nursing Experience in triage or case management
- Two years of experience in a triage or case management position preferred.
- Understand and be committed to the Hospice philosophy of care.
- Excellent assessment and clinical nursing skills and effective interpersonal and communications skills necessary. Computer competency needed.
- Other: Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
