Job Title: Telephone Triage Registered Nurse
Department: Medical / Tigard Administrative Office
Reports To: Nurse Manager / Contact Center

SUMMARY
The Telephone Triage Registered Nurse delivers nursing care to Neighborhood Health Center (NHC) patients via the telephone. This role requires the ability to apply the nursing process (assessment, diagnosis, planning, implementation, and evaluation) to all care provided. NHC uses electronic triage protocols as a tool to assist in triage.
Registered Nurses are expected to understand and follow the Oregon Nurse Practice Act as administered by the Oregon State Board of Nursing and practice according to their scope of practice.
Essential Job Duties
  • Answer incoming phone calls in a timely, professional manner with courtesy and respect.
  • Responsible for the interactive process that occurs over the telephone with the patient.
  • Identifies the nature and urgency of the patient's health care needs and then determines appropriate disposition.
  • Use the nursing process to complete accurate assessments of patients and develops appropriate care plans based on those assessments.
  • Collect and document pertinent medical history during assessment of each patient.
  • Provide guidance to non-clinical Patient Service Representatives (PSRs) during challenging patient calls that require clinical judgement.
  • Schedule an appointment as the patient's condition requires, answer questions, and provide health education in relation to the call as needed.
  • Check voicemail system and answering service messages frequently and route messages to appropriate staff members.
  • Assist in rescheduling patients as needed.
  • Accurately, concisely, and succinctly documents in the medical record all patient contact.
  • Manage an electronic in-basket of incoming and outgoing patient calls, MyChart messages, refill requests, and other messages that directly impact patient care plans.
  • Implements required nursing procedures consistent with NHC procedures/protocol.
  • Participates in process and quality improvement workflows.
  • Remain calm and effective in high pressure and emergency situations.
  • Assist in new employee training on specific tasks, as needed.
  • Maintain punctual and predictable attendance.
  • Perform other duties as assigned.

Education and/or Experience
  • This position requires an Associate Degree in Nursing with 2 years of nursing experience. A Bachelors Degree of Science in Nursing is preferred.
  • This position requires completion from an accredited school of nursing.
  • 1-3 years related experience and/or training preferred in the FQHC or primary care field.
  • Bilingual proficiency in English and Spanish preferred.
  • 1 year of previous medical telephone triage experience preferred.

License and/or Certification Requirements
  • This position requires a current, unrestricted Oregon RN license.
  • Active CPR certification required.
  • National Provider Identifier (NPI) required.

  • Ability to communicate professionally and effectively both orally and in writing with a service-oriented approach, sometimes in stressful circumstances, with patients, coworkers, and the public.
  • Must be able to display sensitivity to the population and patients served.
  • Ability to use good judgement in difficult situations.
  • Ability to proficiently use EPIC Electronic Health Record.
  • Ability to comply with all HIPAA, OSHA, and Neighborhood Health Center policies and procedures.
  • Ability to learn and adapt to changing information and/or policies and procedures.
  • Ability to enter in data accurately with attention to detail.
  • Knowledge of customer service principles and practices.
  • Ability to work in a fast-paced environment.
  • Ability to answer and utilize multiple line telephone.
  • Must be able to demonstrate a record of reliable attendance and punctuality.

WORKING CONDITIONS
Work Environment
  • The noise level in the work environment is usually moderate.
  • As a result of high call volume, this position experiences stressors that come along with a fast-paced environment.
  • While performing the duties of this job, the employee is required to frequently be required to use a headset telephone.

  • Hear and understand the speech of another person.
