General Responsibilities:
- Provide an additional layer of oversight and monitoring to operations leadership to ensure service provision meets the requirements of each individuals’ plan.
- Provide recommendations for the development and monitoring of program and individualized goals and objectives.
- Serve as consultant to regional leadership for problem solving and development of action plans to address quality improvement opportunities identified through internal and external audits and risk assessments with regard to ongoing or unexpected challenges that may affect quality of services.
- Participate in planning meetings for new admissions to the organization, particularly for individuals with complex medical/behavioral needs.
- Serve as a liaison with individual-served medical supports including but not limited to doctors, specialists, pharmacies, medical supply, etc. as needed.
- Comply with all applicable policies, practices, and laws, and with the regulations provided by any governing bodies, such as but not limited to: Health & Human Services (HHS) and Department of Inspections and Appeals (DIA).
- Ensure that agency policies and procedures and regulatory standards and guidelines are implemented appropriately for each individual served within their living environment.
- Ensure all Imagine the Possibilities, Inc. policies and procedures are followed, including the Compliance and Integrity Program and Safety.
- Complete trainings in a timely manner and/or as assigned.
- Report issues of concern to supervisor. Offer ideas and suggestions for resolution of concerns. Use appropriate methods and channels of communication.
- Maintain flexibility to work alternate shifts, locations, weekends, and additional/less hours based on the needs of the agency.
- Other job duties as assigned.
Triage Responsibilities, where applicable:
- Review incident reports daily to triage occurrences when appropriate to ensure appropriate escalation internally and externally. Assist in the creation of a tracking system for triaged events.
- Collaborate with quality team members to track, trend, and ensure proper incident reporting processes and follow up are completed appropriately.
- Ensure priority items identified in the Health Indicator Audit forms have been addressed and completed.
- Participate in discharge planning meetings from hospitalizations or skilled care stays. Ensure smooth transitions and complete follow up care post transition.
- Create and maintain a resource bank with local and regional resources.
- Provide support to team members including attending meetings, data collection, providing resources/trainings and to any other areas identified.
- Assess high needs potential areas of risk for new referrals for services. Evaluate if medical and behavioral support needs can reasonably be attempted to be met. Offer ideas and potential solutions to team members if areas of concerns are identified.
Required Qualifications:
- RN/LPN licensure and in good standing with the state of Iowa preferred.
- Background checks with previous employers, references, law enforcement agencies, and others will be conducted and found satisfactory.
- Valid driver’s license and driver’s record deemed acceptable by Imagine the Possibilities, Inc.
