Seeking a dedicated Home Health RN Patient Wellness Coordinator for a 13-week contract in Wyomissing! Work Monday-Friday, 8 am to 5 pm, promoting patient wellness and utilizing HomeCare HomeBase and Oasis.
Responsibilities:
- Coordinate and provide 1 on 1 clinical care to enhance patient wellness.
- Utilize HomeCare HomeBase and Oasis for streamlined patient management.
- Administer treatments, medications, and educate patients and families.
Requirements:
- Registered Nurse with BLS (AHA) certification.
- Proficient in HomeCare HomeBase and Oasis EMR systems.
- Reliable transportation and a valid driver's license.
- Experience in Home Health.
- 13-week contract.
