```Duties```
- Provide case management services to individuals in need, including but not limited to workforce development, Section 8 housing assistance, crisis management, addiction counseling, behavioral health support, and social work.
- Conduct comprehensive assessments of clients' needs and develop individualized service plans.
- Collaborate with other professionals and agencies to coordinate services and resources for clients.
- Monitor and evaluate clients' progress towards goals and make necessary adjustments to service plans.
- Provide advocacy and support to clients in accessing community resources and services.
- Maintain accurate and up-to-date case records and documentation.
```Qualifications```
- Bachelor's degree in social work or a related field. Master's degree preferred.
- Previous experience in case management or a related field.
- Knowledge of workforce development programs, Section 8 housing assistance, crisis management, addiction counseling, behavioral health support, social work principles, and motivational interviewing techniques.
- Strong communication and interpersonal skills.
- Ability to work effectively with individuals from diverse backgrounds.
- Excellent organizational skills and attention to detail.
- Ability to maintain confidentiality and adhere to ethical standards.
- Valid driver's license and reliable transportation.
