Job description

Join UChicago Medicine, as a Transplant Nurse Coordinator (RN) in our Kidney Transplant Program.

 

As a Transplant Nurse Coordinator (RN), you will provide and coordinate the care provided to patients who have had a kidney, and/or kidney/pancreas transplant. In collaboration with the multidisciplinary team, serves a fully effective member of the team, working alongside the multidisciplinary team.

 

In this role, the Transplant Nurse Coordinator is vital in the delivery of patient care and in the participation of the development of strategies to improve patient outcomes, satisfaction with service and cost effectiveness.  This role is accountable to patients and families for their education, discharge preparation, and follow up/management of their organ transplant. This role is accountable to peers for the advancement of evidence-based practices related to the post-transplant phase of care.

Essential Job Functions

  • Provides patient education and pertinent transplant information to post-transplant recipients.
  • Coordinates meetings with the patients and relevant team members to review the treatment plan
  • Monitors patient progress and answers questions related to their post-operative course
  • Participates in discharge planning with the multidisciplinary team
  • Evaluates patient compliance with their immunosuppressive regimen
  • Orders medication bags and interfaces with transplant financial coordinators to assure appropriate authorization for immunosuppression and other medications
  • Obtains appropriate insurance authorization and/or referrals for medications, diagnostic tests and procedures
  • Provides patient education to patients after discharge for reinforcement of necessary behaviors
  • Works directly with the multidisciplinary care team and coordinates care for post-transplant recipients.
  • Coordinates scheduling of all pre hospitalization medical procedures, diagnostic testing, clinic visits, and medication changes deemed necessary by physician/provider and/or protocol.
  • Communicates with providers concerning patient treatment regimens and related issues.
  • Notifies transplant team members of admissions when appropriate.
  • Coordinates patient admission to hospital when deemed necessary by physician/provider, and or protocol. Provides ongoing post-transplant care management.
  • Under physician/provider supervision, assists patients with medication management, prescription refills, and authorizations as required.
  • Coordinates data collection for post-transplant recipients. Completes the United Network for Organ Sharing (UNOS) forms required post-transplant.
  • Participates in patient-related consultations with physician staff as requested, including mandatory attendance at multidisciplinary meetings/rounds.
  • Communicates with referring physician during the outpatient phase as necessary.

 

Required Qualifications

  • Associate Degree in Nursing; BSN degree or higher preferred.
  • Associate Degree Nurses are required to enroll in a BSN program within six (6) to nine (9) months of hire, and make progress toward and complete their BSN degree within 30 months. ADN nurses receive 100% tuition reimbursement towards a BSN degree from an accredited program
  • Current licensure as a Registered Nurse in the State of Illinois
  • Must possess excellent decision-making and communication skills
  • Must possess effective teaching skills in working with patients and their families
  • Must possess basic computer skills

 

Transplant Nurse  

  • Master's in Nursing preferred
  • Preferred candidates will have five years' experience working with transplant patients
  • A combination of transplant and other related nursing experience may be considered
