We need a Transitional Care Coordinator/Account Liaison to act as a liaison for referrals in Jeffersonville, IN area. Covers Clark, Lloyd, Harrison, and Scott Counties.
As a Transitional Care Coordinator (TCC), you will be called to care when you’re needed most. As part of Interim HealthCare, you’ll support a full range of patient services to bring comfort and dignity to our clients.

As a Transitional Care Coordinator (TCC), you will:
- Serve as a subject matter expert for patients and families moving from a healthcare facility to in-home care
- Act as a patient/family liaison to providers and health plans when navigating the home care transition
- Field customer service inquiries of a clinical nature
To qualify as a Transitional Care Coordinator (TCC) with us, you will need:
- Current unrestricted license to practice as a Licensed Practical Nurse (LPN), Registered Nurse (RN), or Licensed Social Worker (LSW) in the state associated with this position required
- One (1) year of professional experience practicing as an LPN, RN, or LSW, as listed above, required
- Reliable transportation to/from care sites and/or work locations
- Previous experience as a home care intake specialist or case manager preferred
