Job Summary:

We are hiring 1 male and 1 female to provide support to young adult students with Developmental Disabilities, Intellectual Disabilities or Autism who are enrolled in a post-secondary education program. The Advisor for A New Leaf Transition Academy will oversee and advocate for students on his/her caseload and promote the mission and core values of the academy. They will provide consistent professional communication with families and staff, fostering and promoting students personal and professional growth through supports.

 Essential Functions:

  • Provide academic advisement, personal and professional guidance, and career counseling for an assigned group of students through regular individual meetings.
  • Act as the primary liaison and point of first contact between parents of advisees and A New Leaf Transition Academy
  • Interact with instructors/paras, student life and work-based learning staff regarding the academic, living standards and workforce goals of individual students.
  • Manage social issues among students and implement and document strategies within a student’s personal plan as needed, communicate regularly with academic and student life staff regarding student social concerns.
  • Collaborate with parents in an effort to promote student success.
  • Communicate with community professionals regarding medical and psychological health needs.
  • Maintain an electronic file for each student.
  • Develop, implement and execute a counseling schedule.
  • Update student profiles for advisees
  • Complete required documentation, assessments, and communication
  • Assist students with life and workforce skills training (i.e. finances, personal care, and health management)
  • Evaluate and assess prospective students during the admissions process.
  • Contribute to continuous improvement of the program.
  • Attend and assist with staff and student meetings/assemblies/graduations

Responsibilities:

  • Participate in each quarterly student’s Personal Career Development Plan (PCDP).
  • Completes all required documentation and activities not limited to incidents, programmatic data, daily activities, and outcomes.
  • Encourages and assists in the planning of social and recreational activities that are of interest to the individuals being served.
  • Participate in the Transition Academy’s substitute program as needed.
  • Performs duties as requested to meet the needs of A New Leaf Transition Academy.
  • Assist with Work Based Learning Activities, Applied Academic Activities, and Resident and Student Life Activities as needed.
  • Physical capacities must include bending, stooping, lifting a minimum of 30 pounds, as well as the ability to lift, transfer, and position individuals served.
  • Must be able to work with frequent interruptions, deal with diverse personalities during difficult/sometimes stressful situations and still complete job assignments.
  • Must be able to communicate orally, both over the phone and face-to-face.
  • Must be able to prepare and read written documentation as well as perform and understand basic mathematical Functions.
  • Work flexible schedule based on schedules of persons served by and needs of A New Leaf, Inc.
  • Must follow policy and procedures of A New Leaf, Inc.
  • Ensures all individuals served are always safe in their environment and program participation.
  • Must fulfill all training and in-service requirements necessary to work with individuals served within the timeframe required by A New Leaf, Inc., and the State of Oklahoma.

Qualifications:

  • Must be at least 18 years of age.
  • Bachelor’s degree is required; degree in Social Work, Psychology, Human Relations or Special Education is a plus.
  • Must have experience working with individuals with intellectual disabilities.
  • Must have a commitment to student learning and development.
  • Must be an effective communicator, including verbal and written communication.
  • Must possess strong organizational skills
  • Must possess a high level of professionalism, including the ability to maintain confidentiality.
  • Must emulate the values of A New Leaf, Inc., which includes being client-focused, customer focused, professional, and innovative.
  • Must be able to drive, have and maintain a valid Oklahoma driver’s license.
  • Must always have dependable Vehicle.
  • Must always have and maintain automobile insurance coverage.
  • Must maintain eligibility to operate agency vehicles.

Special Requirements:

  • Maintain CPR/First Aid certification. (Sponsored by A New Leaf)
  • Criminal arrest check will be done through the OSBI; Per Section 1025.1 et seq. Of Title 56 of the Oklahoma statues, House Bill 1790, any prospective employee with a felony or misdemeanor conviction, guilty plea or plea of nolo contendere will not be eligible for employment. This prohibition may be disregarded if an appropriate waiver is obtained from the Office of the Director, DDSD, Department of Human Services, or his/her designees.
  • Community Services Registry check (House Bill 1790) will be done through DDSD, Department of Human Services. The person whose name appears on the registry cannot work for A New Leaf.
  • Employees and A New Leaf are engaged in an at-will employment relationship, meaning that either are free to terminate the relationship at any time with or without reason and with or without notice.
