Thp Social Worker I

The THP+FC Social Worker will be responsible for working with up to 12 nonminor (NMD) dependents and/or a ratio of 1 to 8 pregnant of parenting NMD, significant others, placing agency, and adjunct treatment members to enable the NMD to attain his or her goals.

ESSENTIAL DUTIES AND RESPONSIBILITIES INCLUDE THE FOLLOWING:

1. Be knowledgeable of transitional housing programs and resources for transitional-aged foster youth.
2. Be knowledgeable in the Community Care licensing regulations for THP+FC.
3. Conduct a pre-placement appraisal with the placing agency and NMD. Explain the program expectations and complete necessary intake paperwork.
4. Following acceptance into the program the SOCIAL WORKER will obtain intake information and complete a needs and services plan.
5. THP+FC SOCIAL WORKER will complete initial an update the needs and services plan/ STLP and submit to the supervisor within 10 working days.
6. Provide at least weekly consultation to the NMD. Consultation may, but is not limited to discussion of progress towards goals, teaching life skills, teaching parenting skills, public benefits advice, housing advocacy and tenancy training, computer training, transportation assistance, assist in contact with appropriate supportive adults/mentors.
7. To provide assistance with managing the NMD’s cash resources, personal property, and valuables
8. To provide assistance with maintaining accurate records of the NMD’s bank account, savings, and monthly budget.
9. To provide and/or coordinate referrals for additional services, consultants and community resources as needed (i.e. medical, therapy, psychiatric, parenting classes, job training, )
10. Document the NMD weekly progress at home, school, and/or work, major incidents, and other significant, collateral contacts.
11. Complete special incident reports and any other reports as required by the placing agency.
12. Maintain contact with placement worker and inform of any significant events/incidents
13. Consult supervisor regarding incidents, significant issues, and possible terminations, and follow-up as directed.
14. Meet with supervisor for a minimum of one hour weekly of individual supervision and case consultation.
15. Ensure that client files are complete and up-to-date.
16. Submit all reports and documentation to the supervisor for final approval.
17. Facilitate trainings, and support groups as directed by supervisor.
18. Provide on-call crisis intervention. Perform the function of duty-worker at the office as requested.
19. At all times project a favorable image of Walden Family Services.
20. Performs other duties as assigned or requested.

1. Candidate must have a minimum of a master’s degree in social work or a closely related field, as defined in the State Department of Social Services, Community Care Licensing regulations.
2. Must possess a valid California Driver’s License and have a good driving record.
3. Fingerprint and Child Abuse Index clearances from the Department of Justice.
4. Must meet all requirements set by Community Care Licensing about hiring employees by a licensed community care facility.

EMPLOYEE STATUS: Exempt

SALARY RANGE: $66,560
