About this position

The MTM Pharmacist Supervisor position at North Memorial Health combines supervisory responsibilities with patient care activities in the context of a certified Health Care Home (HCH) environment. The primary focus of this role is to lead a team of pharmacists and non-clinical staff in providing Medication Therapy Management (MTM) services within an ambulatory care setting. MTM aims to comprehensively assess a customer's medication regimen, optimize medication therapy, and collaborate with the primary care team to enhance health outcomes for individuals with chronic health conditions. While overseeing the team, the supervisor will also actively engage in patient care, spending approximately half of their time directly assisting customers.

 

Requirements

Education

Doctor of Pharmacy: Graduated from an accredited college of Pharmacy OR have successfully completed MTM certification
MTM residency preferred but will consider equivalent combination of Education and MTM Experience
Board Certified Ambulatory Care Pharmacist (BCACP) highly desired
   

Experience

1+ year experience required in MTM residency or previous MTM position, additional experience is preferred
Knowledge, Skills and Abilities
Must possess excellent oral and written communication, leadership, self-initiative, innovative, analytical and collaborative skills
Builds effective partnerships with providers and clinic staff fostering open lines of communication and establishing trust within the HCH team based care model
Evaluates progress towards attainment of goals or outcomes
Electronic Medical Record familiarity with EPIC is highly desired  

Licensure/Certification

Licensed by MN State Board of Pharmacy
BLS Certification
Board Certified Ambulatory Care Pharmacist (BCACP)
