Summary
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

The Primary Purpose of the Position is: The Silver Team Clinical Team Leader Supervisory Social Worker (ST CTL Supervisory Social Worker) comanages the operations of the Silver Team, a mental health Geri psych outpatient program.

The Silver Team Clinical Team Leader Supervisory Social Worker comanages operations of the Silver Team, duties include but are not limited to:
  • The incumbent reviews all data, subjective and objective, and makes a clinical assessment identifying needs and strengths. The incumbent interprets data and to identify viable treatment options. The incumbent assesses high risk factors, acuity, and need for services.
  • The incumbent serves the Veteran, who tends to have frequent and severe crises, lack family or an adequate community support network, may fail to follow through with instructions and treatment plans, or have significant deficits in coping skills and require continuing professional psychosocial support.
  • The incumbent uses professional judgment and advanced practice skills, based on the psychosocial assessment, to make a psychosocial diagnosis. The incumbent assesses at-risk factors and develops a preliminary disposition plan involving the Veteran and as appropriate the Veteran's family and/or significant other. The incumbent performs assessments of serious and complicated cases involving psychiatric illness involving psychiatric, catastrophic medical conditions, dementia and other high-risk diagnoses.
  • The incumbent finds a suitable means of treatment to help Veterans and/or significant others to cope with stressful situations.
  • The incumbent gives advice, guidance, emotional support, and other assistance. The incumbent provides individual and group counseling services, including psychotherapy in some settings.
  • The incumbent makes provision for referral services from other agencies, community resources, and other Governmental agencies to resolve problems.
  • The incumbent treats Veterans with complex problems, histories of non-compliance with treatment and inability, or unwillingness to participate in treatment.
  • The incumbent exercises professional control over the therapeutic relationship so the patient may constructively express feelings yet retains maximum use of his/her capacities for responsible self-direction.
  • The incumbent develops effective and strong relationships with community resources to ensure continuity of care and to promote successful transitions of services.
  • The incumbent receives and completes requests for services (consults) from interdisciplinary team members and from other Social Workers on complex, difficult cases, using advanced practice skills and expertise.
  • The incumbent provides case management services and interventions to Veterans.
  • The incumbent advocates for the Veteran and as appropriate the Veteran's family, caregiver and/or significant other.
  • The incumbent provides education related to the Veteran's VA entitlements, eligibility, services, and programs, as well as community resources for medical and mental health needs.
  • The incumbent educates the Veteran and as appropriate the Veteran's family, caregiver and/or significant other about VA Advance Directive: Durable Power of Attorney for Health Care and Living Will and the Mental Health Advance Directive.
  • The incumbent coordinates referrals and consults for VA services and services from community-based agencies in collaboration with the interdisciplinary team members. The incumbent coordinates referrals of VA placements in Inpatient Behavioral Health Programs, Homeless Domiciliary, Adult Homes, Assisted Living Programs, Substance Abuse Programs, CNH, NHCU, and Residential Rehabilitation Programs etc...
  • The incumbent develops and maintains treatment and documentation standards that meet the expectations and requirements of Joint Commission and CARF accreditation.
  • The incumbent independently identifies high-risk patients and provides crisis intervention services.
  • The incumbent completes suicide risk screens and documents assessments as required by policy.
  • The incumbent provides wellness/prevention education and facility patient and family support groups as appropriate.
  • The incumbent provides supervision to staff attached to the Silver Team. Tasks include, work assignments, monitoring of productivity, Ongoing Professional Practice Evaluations, chart audits, and evaluations.???????

Basic Requirements:
Citizenship: Be a citizen of the United States. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA policy).
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website http://www.cswe.org/Accreditation to verify if that social work degree meets the accreditation standards for a master of social work.
Licensure: Persons hired or reassigned to Social Worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM website http://vaww.va.gov/OHRM/T38Hybrid/.
English Language Proficiency: Social workers must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f).
Grade Determinations: Supervisory Social Worker GS 13.
Experience/Education: One year of experience equivalent to the GS-12 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidates may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification. Individuals assigned as Social Worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities (KSA). In addition to the experience above, the candidate must demonstrate all the following KSAs. It is recommended to provide a written narrative format of the subparagraphs below:
(a) Ability to independently organize work, set priorities, and meet multiple deadlines.(b) Skill in a range of supervisory duties which includes clinical supervision, consultation, negotiation with other departments and quality improvement.(c) Ability to ensure provision of clinical social work services by supervised Social Workers.(d) Ability to delegate authority, manage priorities, coordinate work, and follow up on pending issues to complete duties in an accurate and timely fashion.(e) Ability to analyze organizational and operational problems, develop and implement solutions that result in efficient operations, and use data effectively to manage workload, quality, performance, and productivity within the service.(f) Ability to provide staff training and development.
Preferred Experience:
  • Full, unrestricted License Clinical Social Work license.
  • Experience treating Veteran population.
  • Experience working with Geriatric population.
  • Experience working with or managing a mental health outpatient unit.
  • Experience working with or managing a mental health acute Geri-psych unit.
  • Experience working with elderly Veterans with complex discharge cases.
