Form of workWe have a wonderful opportunity for a Community Based Supervisor at our Syracuse, NY location.
Full time position (40 hours) Monday to Friday.
The Community Based Supervisor is responsible for the quality of services to clients and the performance of the Homefinding Specialists and Permanency Specialists in a Community Based/Residential setting. The Community Based Supervisor supports the Homefinding Specialists and Permanency Specialists in order to meet the needs of clients. The Community Based Supervisor is responsible for the efficiency of the day to day operations of the program including scheduling staff, hiring, training, regulatory compliance (OCFS, OPWDD, OMH, JJS, COA, CMS, etc.,), and safety of youth, families and staff. The Community Based Supervisor coaches Homefinding Specialists and Permanency Specialists and is responsible for the efficiency of day to day operations of the program including scheduling staff, hiring, training, regulatory compliance and safety of youth, families and staff. The Community Based Supervisor conducts Individual and group supervision of Homefinding Specialists and Permanency Specialists reviews Home studies and all related assessments as part of the foster parent certification process, in partnership with the
Responsibilities:
- Manage a staff of up to 15 people
- Manage multiple services and locations
- Supervise Skill Builders to ensure delivery of comprehensive family driven services within the communities they serve
- Oversee staff schedules to ensure adequate coverage and oversee program services
- Ensure that the team meets on a regular basis and maintains accurate minutes of such meetings as well as other team decisions and processes
- May be required to assume the responsibility of the Program Manager - Community Based in their absence
- May be required to perform all Skill Builder functions as needed
- Be an active participant in agency and team meetings, volunteering and serving on agency and community committees as needed
- Attend and participate in required meetings (inclusive of supervisory training and educational opportunities for further professional growth)
- Communicate on behalf of the team with families, internal and external resources and funders
- Provide 24-7 back-up to Skill Builders who perform crisis intervention and response
- Ensure that services provided meet the Medicaid requirement for medical necessity, are linked to the goals of the service plan ("golden thread") and that the documentation meets all regulatory requirements
- Ensure that all skill builders provide the budgeted number of billable units on a weekly basis and provide data necessary to support Medicaid billing
- Gather information and create a bill for all contracts with external funders
- Provide a level of Care Coordination for cases for which SB is the only service (both Intensive, customized and UW)

Requirements:
- Bachelor's Degree (BS) in Human Services or related field (equivalent combination of education and experience may be considered in certain programs)
- Driver's license with clean driving record
- 2+ years of experience in Human Service
- 3+ years of supervisory experience is preferred but not required
$22.01 Minimum Rate per hour, $33.01 Maximum Rate per hour, based on experience.
