- The Substance Use Disorder Counselor is responsible for providing a wide range of confidential addiction and mental health services, education, and support to individuals in the communities we serve. The counselor coordinates efforts withinternal team members, and external substance use / mental health agencies.
Essential functions:
Provide direct care in determining patients' status and issues by interviewing them, obtaining personal information, and examining any previous treatment record, to assess program readiness and the most appropriate treatment program to help clients access the right level of care to meet best their treatment needs (i.e., IOP, in patient, and more).
Work closely with other healthcare professionals to treat problems requiring a multi-disciplinary approach.
Design and manage a personalized recovery plan for each patient. Provides, at each visit, updated, detailed goals and treatment plans targeted to specific substance abuse issues.
Update treatment plans based on information gathered during individual or group therapy.
Please become familiar with services and resources in the community, and work closely with them to facilitate referrals when needed.
Coordinate care with community-based organizations and agencies, appropriate to restoring patients to productive roles.
Perform clinical and administrative practices that comply with confidentiality requirements and staff and federal regulations.

Monday, Tuesday, Wednesday & Friday, 8:00 AM - 5:00 PM; Thursday, 10:00 AM - 7:00 PM
No weekends, 8 Paid Days OFF
