Street Medicine Outreach Specialist
STRIDE Community Health Center is a 501(c)(3) Federally Qualified Health Center (FQHC) with a 30-year history of providing primary care and other health-related services in a community-based setting. Established in 1989 in response to the primary health care needs of the medically indigent population in suburban Denver, STRIDE Community Health Center has the largest population base of any FQHC in Colorado and provides primary care, dental, pharmacy, behavioral health, health education, care coordination, patient navigation, pharmacy, and outreach and enrollment services in accordance with its mission.
The Street Medicine Outreach Specialist partners with their medical teammate to meet the needs of our patients directly in the community. This position will be working outside of the traditional clinic setting, working with patients of all backgrounds to ensure everyone receives the care they need with dignity and respect. This unique opportunity will both ensure patients are properly registered within Strides system and help community members navigate financial and community resources.
Essential Duties and Responsibilities
- Demonstrates professional behavior with the public, patients, and peers at all times
- Registers, check-in, and checks-out patients for all services
- Maintains charts, and ensures Health Insurance Portability and Accountability Act (HIPPA) and OSHA compliance while demonstrating understanding of regulation
- Collects and uploads all necessary documentation and consents
- Promptly and accurately updates patient records including financial and demographic information
- Promptly coordinates scheduling of new patient referrals, gathers necessary information, and schedules appointments accordingly
- Collects copayment and account balances at time of service
- Acts as liaison between patients and Billing Department regarding charges, payments, and insurance
- Schedule’s appointments according to the scheduling guidelines
- Assists in Medical Records duties as needed
- Works closely and communicates consistently with internal staff to ensure continuity of care
- Supports other STRIDE clinics based on the needs of the organization
- Researches and verifies patient eligibility for different programs and insurance coverage
- Registers and interviews patients to obtain accurate and complete demographic and financial information.
- Informs patients about the availability of governmental funded and discount programs.
- Properly completes enrollment applications through electronic and manual systems.
- Responsible for identifying and correcting enrollment discrepancies.
- Ensure the protection and security of personal, confidential, and identifiable information in a professional and responsible manner and carry out all measures to prevent any unauthorized disclosures.
- Maintain knowledge and expertise in program eligibility, enrollment, and program specifications of Medicaid, Child Health Plan +, Medicare Savings, Title X, RWG, SDP, STRIDE Discount and Connect for Health Colorado.
- Understand, interpret and communicate federal, state and eligibility criteria, policies and procedures.
- Works collaboratively with the Patient Accounts team to ensure timely account adjustments.
- Records and communicates the outcome of the screening to the patient.
- Coordinates wrap-around services for patients, including collaborating with clinical and other staff, to address patient needs in a variety of settings as appropriate.
- Knowledgeable about appropriate STRIDE and community resources to effectively address needs of patients/families identified health concerns.
- Assists in identification of risk factors leading to increased utilization of healthcare resources and works to reduce preventable hospitalizations, readmissions, and Emergency Department visits and communicates these risks to Care Team.
- Collaborates with medical, dental, and behavioral health providers to help ensure patients have comprehensive and coordinated care. Follow-up with patients including documentation and communication to other care team members should be continuous from initial identification through closure.
- Act as a patient advocate and liaison between patient/family and community service agencies (i.e., schools, Department of Human Services, home health agencies, specialists, hospitals, etc.).
- Record patient care navigation and goal setting in the EMR within 48 hrs. of encounter.
- Help patients connect with transportation resources and give appointment reminders in special circumstances as needed. Transporting patients is prohibited.
- Determines appropriate internal and external service referrals, ensures services are received, and monitors results.
- Process internal and external referrals daily, prints and faxes pertinent patient health information to specialists according to STRIDE and HIPAA guidelines.
- Enters referral information into the database.
- Follows up on all referrals following guidelines.
- Speaks to specialist offices, as necessary, to assist in the guidance of the referral process and problem solve any concerns that may arise as a result of the referral process.
- Pulls and/or requests reports for the specialist to close out the referral.

Education and Experience
Education or Formal Training: High school diploma/GED required (Associate degree preferred)
Experience: One to two years related experience and/or training; or equivalent combination of education and experience. 1+ years’ experience working front desk in a medical or dental setting preferred.
Knowledge, Skills and Abilities
- Ability to interact positively and build rapport with patients, coworkers and/or external contacts.
- Ability to work independently and organize work in a manner that ensures accuracy and efficiency.
- Skill in using a variety of office equipment including but not limited to postal machines, printers and fax/scanners.
- Ability to follow directions.
