Objective
Provide assistance with the coordination of access to health care services that includes connecting Veterans to VA health care benefits or community health care services where Veterans are not eligible for VA care. Responsibilities include but are not limited to, case management, care coordination, patient education, consultation, administrative duties, and direct follow-up related to accessing and/or retaining health care and supportive services. Work closely with the Veteran's primary care provider and members of the Veteran's assigned interdisciplinary treatment team.
Essential Functions, Duties & Responsibilities
- Act as a liaison between Volunteers of America Michigan (VOAMI) and the VA or community medical clinic and works with a population of Veterans with complex needs who require assistance accessing health care services or adhering to health care plans
- Coordinate referrals to VA, community health clinics, and other programs needed to ensure access to health care
- Provide services designed to enhance client health and independent living skills, thereby promoting client housing stability
- Advocate for Veteran's needs and assist in identifying concerns or questions about their treatment or medications to develop open communication with the provider or treatment team
- Work closely with the Veterans assigned multidisciplinary team, including medical, nursing, and administrative specialists, and case management personnel
- Provide timely, appropriate, Veteran centered care equitably
- Work collaboratively with the team and the Veteran to identify and address systems challenges for enhanced care coordination as needed
- Conduct Non-Clinical Assessments
- Provide ongoing health education support as needed to the Veteran and family member
- Collaborate with other disciplines involved in providing care
- Identify systemic barriers within the organization, communicates with organizational leadership about these barriers, and work collaboratively to find viable solutions
- Assist in developing policy, procedures, and practice guidelines related to the specialty program using knowledge gained from research or best practice
- Develop evaluation components and outcomes indicators and report those evaluation results to VA and organizational leadership
- Embrace and embody the mission, vision, guiding principles, clinical vision and goals of VOAMI
- Maintain compliance with VOAMI policies and procedures
- Ensure that all duties are carried out in a timely and orderly fashion so that VOA Michigan functions in the most efficient manner possible
- Perform all other duties as assigned
Qualifications (Education and Experience):
Required:
- Licensed Master's level social worker, psychologist, or professional counselor from an accredited university
- Minimum of two years of experience in a healthcare or social services area
- Successful completion of the probationary period
- Possession of a valid Michigan Driver's License and an operable insured automobile for authorized travel
- Must have a driving record that is acceptable to VOAMI's insurer
Knowledge, Skills and Abilities
- Knowledge of available community resources, human service agencies, programs and services available for veterans
- Demonstrated experience working with diverse cultures and populations
- Ability to establish and maintain effective relationships with co-workers, clients, community partners, and the public
- Ability to gain knowledge of healthcare systems and operations, local healthcare options and resources including mental health, and community resources and government assistance programs for the underserved
- Comfortable with frequent interaction with clients, physicians, and other hospital personnel and community partners
- Ability to define problems, collect data, establish facts, and draw valid conclusions
- Ability to work independently and meet deadlines with minimal supervision
- Ability to organize and manage time wisely
- Ability to effectively speak, write and understand the English language
- Ability to speak and understand a language other than English is preferred
- Ability to conduct oneself with tact and courtesy
- Ability to work outside of normal business hours as determined by supervisor need
- Must have demonstrated skills in the MS Office 2000 Suite or higher (includes WORD, EXCEL, POWERPOINT, EMAIL, OUTLOOK and Internet Explorer)
