Responsibilities

Enhance clinical quality and safety of medication use process
 Develop and implement drug use and/or medication safety guidelines/protocols/policies.
 Critically evaluate new drug requests to the formulary.
Develop and conduct medication use evaluations (MUEs)
Coordinate, prepare, and present information for multidisciplinary (P&T, Patient Safety, Subcommittees,
etc) committee review and approval.
 Monitors success of policies/procedures, makes recommendations to improve performance, and
implements changes as needed
Presents medication and quality -related data internally and externally to various audiences.
Help establish medication safety performance measures.
Coordinate and/or aid in the collection of regulatory and other safety data aimed at driving performance
improvements.
Completes and/or coordinates assigned nursing unit and clean & safe inspections.

Provides ongoing assessment of the formulary and takes action as needed.
Maintain expert knowledge of regulatory requirments, goals, and standards in relation to medication
management
Effectively demonstrate the principles and conduct consistent with a High Reliability Organization.
Manage Pharmaceutical Expense for Target Therapies and/or Clinical Areas
Report on the status of drug target performance and special projects (e.g. Drug Use Optimization,
subcommittee agendas, etc) to the Associate Director at least once a month)
Identify new opportunities for cost reduction, containment, and/or revenue enhancement

Aid in the preparation of new drug technologies and budget forecast, including changes in clinical
services, volume expectations, and spend rate analysis
Coordinate and or lead key stakeholder meetings to secure buy-in on cost-reduction strategies
Mentor and develop pharmacy staff
Supports the ongoing development of clinical staff by providing continuous performance feedback.
Assist staff in development of projects such as MUEs, formulary evaluations, and drug information
publications/questions and presentations.
Serve as a resource to staff for questions on policy, procedures, problem resolution, etc.
Bring ideas and issues to the leadership team with the goal of enhancing staff engagement
Lead staff engagement groups

Provide feedback to leadership on succession planning
Serve as Clinical and Operational resource to assigned areas
Meets with area staff to discuss and resolve operational problems/areas in need of attention
Provides each pharmacy customer (patients, physicians, nurses, etc.) with appropriate, considerate,
and respectful service.
Assists with patient care and operational staffing needs as needed.
Makes appropriate interventions aimed at improving patient care, quality, and/or cost-effectiveness.
Performs regulatory audits as indicated
Performs responsibilities assigned to the designated shift being covered.
Provides feedback to leadership team on staff/shift enhancements to improve work flow efficiency as
identified.
Documents interventions
Participate in hosptial committees in designated specialty areas (e.g.Med Ops, M&M, tumor boards,
Patient safety, ICU Leadership)
Develop educational initiatives
Identify educational needs of staff in specialty area
Develop and deliver effective educational material (presentations, inservices, handouts, policies, etc)

Assess effectiveness of education as measured by staff performance.
Develop educational plan for target areas of responsibility


Qualifications

EDUCATION (number of years and type required to perform the position duties):
Graduate from an accredited College of Pharmacy with a B.S., M.S., or Pharm. D. degree. Completion of a
residency in hospital pharmacy from a program accredited by the American Society of Hospital Pharmacists is
highly desired but not required.


EXPERIENCE (number of years and type required to meet an acceptable level of performance):
Minimum of three years of experience in a clinical practice setting with demonstrated skills in formulary evaluation , drug information, and project management. Experience in leading or participating in medical committees and
application of quality improvement techniques is required.


SPECIAL SKILLS: Experience in drug utilization review and data analysis (e.g. MS Excel, MS Access) is highly desirable. Excellent
written and verbal presentation skills are required. Should be familiar with Change Acceleration Process (CAP) or
similar process improvement/change management skills.


ACCOUNTABILITY (how this position is held accountable for such as goals achievement, budget adherence, or
other areas of accountability):
The individual in the position is held accountable to the Manager, Drug Use Policy for maintaining the highest of
professional standards, promoting rational and cost-effective therapy and establishing safe medication practices . The Sr Clinical Pharmacy Specialist will be responsible for the financial performance of specific drug or drug class
targets, meeting regulatory and other governing body standards of medication management for assigned area, and
managing quality outcomes for specified area or processes.

possess a superior knowledge of current standards of pharmacy practice.
