Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children and families in three main areas:

  • Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Migrant Seasonal Head Start, and Child Development Center.
  • Clinical Health Services: Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.
  • Supportive Health Services: Family Stabilization, a Teen Outreach Program, SOAR, Youth Leadership Program, Youth Shelter and Housing Assistance Programs.

Spravato Coordinator

Job Title: Spravato Coordinator

Reports to: Psychiatric Services Nursing Manager

Areas of Responsibility: Manage and maintain the Spravato/case management roles and responsibilities designed to provide psychiatric and mental health care. Case Management efforts will focus on evidence-based practice and social determinant support of patients with psychiatric or mental health disorders. The Spravato/Case Manager will also work with psychiatry, nursing, and other clinical and non-clinical staff as needed.

Spravato Coordinator Responsibilities: Manage and maintain a case management program designed to provide psychiatric and mental health care as well as follow up case management for patients. Case Management efforts will focus on evidence-based practice and social determinant support of patients. The Case Manager will also work with psychiatry, nursing, and other clinical and non-clinical staff as needed. This will include:

  1. Spravato Insurance Prior Authorization Coordination
  2. Responsible for scheduling initial Spravato treatment
  3. Responsible for ordering, inventorying, and disposal of Spravato per Spravato protocols. Inventory and disposal must be witnessed by an additional staff member.
  4. Act as a patient advocate and practice effective patient communication; assist in case management, patient population management, and test tracking as well as supporting patient care by coordinating services with both community resources and other Community Action Partnership of Western Nebraska (CAPWN) program areas.
  5. Utilize the Electronic Medical Record (EMR) to assist in the delivery of quality care and to track patient’s on-going care.
  6. Generate and analyze clinical data to identify patients who require screening and are appropriate for the program with input from the Spravato providers.
  7. Provide regular reports to staff tracking progress of relevant measures.
  8. Provide patients with education and information (via phone, email, or mail) about Spravato treatment and Spravato program guidelines.
  9. Operates the computer software associated with the REMS program and other computer programs utilized to support the Spravato therapy and treatment.
  10. Completes a prescreening tool per Spravato protocol and if additional screening is required it can be requested by the Spravato provider.
  11. Provides the CAHC Spravato Providers with a weekly report of the patient clinical status (including overall progress, side effects, medication changes, or other new clinical data) for each patient receiving Spravato treatment.
  12. The Spravato Coordinator/case manager will interrupt treatment, if the pt expresses concern, and will contact the Spravato treatment provider to inform of the patient’s issue. The Spravato treatment will resume or stop based on the Spravato provider’s assessment and order.
  13. 14.Attend Monthly QA/QI meetings as psychiatry team representative.

Working Conditions:

  • Clinic environment trainings, meetings, and community interaction.
  • Well-lit environment with standard medical and office equipment available.
  • Work is frequently dynamic and requires meeting tight deadlines.
  • Work in a highly volatile program in a state of change with many interruptions and interactions.
  • Must be able to work irregular hours, some weekends and evenings and out-of-town training.
  • Fast paced working environment

Physical Requirements:

  • Ability to frequently bend, stoop, squat, walk and pull drawers.
  • Manual dexterity sufficient to operate office and medical equipment.
  • Must regularly talk, hear, and have close vision ability to record and communicate appropriate documents and records.
  • Occasionally lifting and carrying up to 25 pounds. Able to assist patient to and from wheelchair and to the restroom.

Minimum Qualifications:

Grade: 11

  1. High School Diploma/equivalent with three years’ experience in clinical procedures, data entry and/or medical assisting, preferably in health clinic setting and three years’ management experience.
  2. Requires proficient level of communication oral and written.
  3. Requires proficient level of computer knowledge with ability to work with Microsoft office program spreadsheets and reports.
  4. Proficient level of teamwork and managing conflict, ability to recognize and delegate appropriate tasks and develop training.
  5. Valid driver’s license and good driving record.
  6. Age 21 or over.

Grade: 12

  1. Must meet minimum qualifications above.
  1. Five years of above experience, preferably in a health clinic setting, with five years’ management experience.

Grade: 13

  1. Must meets minimum qualifications above;
  2. Seven years of above office experience, preferably in a health clinic setting, with seven years’ management experience or specialized training.
