Are you a Speech Therapist who wants a flexible schedule and work-life balance? Synaptic Rehab is seeking a dynamic and passionate Speech Therapist to join our team on a per diem basis. As a Speech Therapist - Home Health, you will provide professional speech therapy services to patients in their homes, while having the flexibility to work around your personal schedule.
Responsibilities:
- Assess patients' communication and swallowing abilities and develop individualized treatment plans
- Provide speech therapy services to patients in their homes
- Monitor and document patient progress towards goals
- Communicate effectively with patients, family members, and interdisciplinary team members
- Participate in interdisciplinary team meetings to discuss patient progress and plan of care
- Maintain accurate and up-to-date patient records
- Adhere to all ethical and legal standards for speech therapy practice
- Participate in ongoing education and professional development activities
Qualifications:
- Master's degree in Speech-Language Pathology from an accredited program
- Current state licensure as a Speech-Language Pathologist
- Certification of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred
- Minimum of one year of experience in speech therapy, preferably in a Home Health setting
- Excellent communication and interpersonal skills
- Ability to work independently and as part of an interdisciplinary team
- Strong organizational and time management skills
Physical Demands: The Speech Therapist must be able to travel to patients' homes and provide therapy services in various home environments. The therapist must be able to lift and carry equipment and materials as needed for therapy sessions.
