About This Home Health Speech Therapist Position

HopeHealth is seeking an experienced Speech Language Pathologist (SLP) for its affiliate HopeHealth Community VNA to visit patients in southern Massachusetts.

This position will be headquartered in Attleboro, MA.

This is a part-time 24-hours per week position that is eligible for benefits.

This team will service Southern MA

What will you do?

As a home health speech language pathologist, you will provide one-on-one therapeutic treatment and care to patients in their home using a comprehensive and patient specific care plan, innovative solutions, and a point of care documentation.

You will have the opportunity to spend quality time with the patient, in their own home, to promote their health and independence. You will perform evaluations and interpret symptoms in order that appropriate treatment techniques can be implemented.

The primary focus of this is to access and treat patients with speech, language, swallowing, cognitive and voice disorders.

• Knowledgeably and competently delivers speech therapy to patients in their homes.
• Evaluate patient needs through ongoing assessment and revisions to care plans based on changes in patient condition.
• Follows the multi-disciplinary plan of care
• Regularly re-evaluate patients to identify progress towards goal achievement; completes OASIS assessments and coordinates with Quality to ensure proper coding and scoring; obtains orders and updates Plans of Care as needed throughout the episode of care.
• Based on this assessment and evaluation, develops speech therapy treatment plan of care or programs tailored to meet the specific needs of the patients within their home setting.
• Participates effectively as part of an interdisciplinary team to assure that patient services are delivered in a timely manner.
• Documents patient care and data accurately, thoroughly with point of service documentation but no later than end of work day.
• Secures positive clinical and quality outcomes according to state and national benchmarks.
• Case management and collaborate with interdisciplinary teams.
• If needed participate in weekend and holiday rotation.
• Facilitate and participate in case conferences.

What are the qualifications for this job?

• Master's degree in speech language pathology from an accredited program in Speech/Language Pathology.
• Minimum of one year speech therapy experience, preferably in homecare
• Current professional license to practice Speech Therapy in Massachusetts
• Ability to provide independent speech therapy based on sound scientific principles and intelligent professional judgment.
• Valid driver's license, automobile insurance, and reliable transportation to travel to patient homes.
