Duties

The RN Case Manager:
  • Takes responsibility for Outpatient clinic coordination and patient flow management, including appointment management, triage of patients, patient education, monitoring of patient progress toward predetermined goals and timely follow-up.
  • Participates in clinic for setting patient goals. Is part of the interdisciplinary team. Utilizes the nursing process in managing, developing, implementing and evaluating each patient's individual plan in collaboration with the interdisciplinary care team to attain specific clinical outcomes in the outpatient specialty clinic settings.
  • Provides support to providers daily: coordinating dual care with community and other VA facilities; ensuring all needed records, tests and other information is available to provider at time of consultation to allow provider to focus on treatment plan and Veteran's questions in an appropriate and timely manner.
  • Receives and triages consults with the specialty service for one of the following: I) Provide Scheduling Direction 2) Convert to an E-Consult, providing ordering provider with recommendations and test/images 3) Forward to Community Care for authorization and scheduling
  • Communicates with Veterans their health care options including local VA, other VA Medical Centers and the community based on eligibility criteria for community care per the National Directives (ex. MISSION Act).
  • Works closely with other VA Medical Centers, contracted facilities and community facilities to remove obstacles that would impede the Veteran from utilizing the services of other VA medical centers or community not available at our facility.
  • Participates in activities which meet the local and national performance standards contract for the Medical Center.
  • Participates in the development and implementation of recommended policies and procedures as related to Case Management and/or designated clinical area.
  • Provides for the educational needs of the health care team regarding Case Management and/or topics specific to designated clinical care.
  • Maintains and upgrades own knowledge base and skills regarding Case Management and/or specific clinical areas and applies this knowledge to nursing practice.
  • Participates in clinical research/EBP within designated area and/or related to Case Management.
  • Establishes effective networking relationships with peers and professionals within the community as related to Case Management.
  • Receives and triages new patient consults, assists with ordering and coordinating appropriate tests, and communications with the referring provider, private institutions, and physicians, to expedite patient appointments.
  • Collaborates with clinic staff to ensure patients are given timely appointments and expedite new patient consults.
  • Follows up with patients, monitoring and ensuring adherence to treatments plans, intervening in accordance with prescribed clinical guidelines and providing reports on all cases.
  • Provides clinical care and case management to Veterans in the Specialty Clinics.
  • Triages and applies a collaborative team approach in identifying, analyzing, and resolving patient care problems.
  • Communicates and collaborates with the interdisciplinary team to develop and coordinate the treatment plan for Veterans in the outpatient setting.
  • Serves as a resource for other team members and participates in planning for program improvements at the local level.
Each Specialty Service is set up independently based on volume/size and interdisciplinary team members assigned within the service. The above scope is included within but not limited to each service. Expectations and Scopes change continually based on specialty service, service volume, team members, National Directives/Mandates, etc. to meet the needs of the Veteran

    Qualifications
    Basic Requirements:
    • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
    • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing ( ACEN ) or The Commission on Collegiate Nursing Education ( CCNE ). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
    • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
    Preferred Experience:
    • BSN preferred
    • 3-5 years Nursing experience preferred
    • 3-5 years Outpatient experience preferred
    • 3-5 years Nurse case management experience preferred
    Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
    • Nurse I Level I - Diploma or associate degree in nursing with no professional nursing experience.
    • Nurse I Level II - Diploma or associate degree in nursing and one year of specialized nursing experience equivalent to a Nurse I, Level 1; OR a Bachelor's degree in nursing and no professional nursing experience.
    • Nurse I Level III - Diploma or associate degree in nursing and two years of professional nursing experience, one of which is a year of specialized nursing experience equivalent to Nurse I, Level 2; OR a Bachelor's degree in nursing and one year of professional nursing experience equivalent to Nurse I, Level 2; OR a Master's degree in nursing and no professional nursing experience; OR a Master's degree in a related field with a bachelor's degree in nursing with no professional nursing experience.
    • Nurse II - Bachelor's degree in nursing and two years of professional nursing experience, one of which is a year of specialized nursing experience equivalent to Nurse I, Level 3; OR a Master's degree in nursing with one year of specialized nursing experience equivalent to Nurse I, Level 3; OR a Master's degree in a related field with a bachelor's degree in nursing with one year of specialized nursing experience equivalent to Nurse I, Level 3; OR a Doctorate degree in nursing with no professional nursing experience; OR a Doctorate degree in a related field with a master's or bachelor's degree in nursing with no professional nursing experience.
    • Nurse III - Master's degree in nursing with two years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Master's degree in a related field with a bachelor's degree in nursing with two years of specialized nursing experience, one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Doctorate degree in nursing with one year of specialized nursing experience which is equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Doctorate degree in a related field with a master's or bachelor's degree in nursing with one year of specialized nursing experience equivalent to Nurse II and meets all dimension requirements for Nurse III.

    Physical Requirements: Walking, bending, stooping, and some lifting in the direct care of patients. There may also be extended periods of sitting.
