Job description

JOB SUMMARY

The Specialized Habilitation Stabilization provider works directly with clients (children and adults) with DD or ID that reside in their homes, assisted living facilities, group homes, or long term care facilities and need support with life goals. This position is responsible for stabilizing challenging behaviors, promoting inclusion in the community and providing services individualized to the client. Support shall be provided with the intent of reaching a habilitative goal identified through consultation with the clients PNW Roots Counselor and PNW Roots Support Team members.

QUALIFICATIONS

- Must have a minimum of one (1) year experience working with individuals who experience a developmental or intellectual disability.

- Must have a Bachelors or higher Degree in Psychology, Social work and/or Education.

JOB SKILLS/TRAINING/EQUIPMENT

Excellent communication skills, both verbal and written. Ability to work with a diverse client/family population in a variety of settings. Ability to work independently and as a team member.

ESSENTIAL JOB FUNCTIONS

Provide support to address up to three goals under one or more of the following Specialized Habilitation domains :

1. Self-Empowerment Support to increase self-esteem, confidence and develop skills to achieve personal development goals.

2. Safety Awareness and Self-Advocacy Support to increase safety awareness and effectively express desires or needs.

3. Interpersonal Effectiveness and Effective Social Communication Support to learn and develop social skills used to build and maintain friendships or increase acceptance in the community or at home.

4. Coping Strategies Regarding Everyday Life Challenges Support to improve problem solving skills and reduce stress associated with unexpected or unavoidable situations.

5. Managing Daily Tasks and Acquiring Adaptive Skills Support to develop the skills necessary to reside successfully and increase independence in the home and community.

-Deliver services and support in community-based settings, which enable an individual to attain the maximum functioning level or to be supported in such a manner that allows an individual to gain an increased level of self-sufficiency.

-Update the Specialized Habilitation Plan goals with input from the client annually or more often if needed when requested by the client.

-Develop and implement a written Specialized Habilitation Plan for each client that includes, at a minimum, the following:

1. Specific, measurable, observable, time-limited goals, and objectives (no more than 3 at a time);

2. Specialized Habilitation service and subcategory being targeted, including the proposed frequency and duration of service and methods to be used;

3. Treatment strategies to achieve the goal;

4. A description of how Client progress toward goals will be assessed or measured;

5. Goal completion criteria stated in objective, measurable terms; and

6. Client signature indicating participation in plan development.

1. Specialized Habilitation Plan SMART Goals and Objectives:

a. Specific – what to achieve, how to achieve, why to set the goal.

b. Measurable – focus on the amount, frequency, or timeline.

c. Attainable – within reach and not too difficult to achieve.

d. Relevant – based on input from the client and related to needs identified in the PCSP.

e. Timely – be realistic to help measure progress and keep focused on the development of the individual.

2. Current treatment strategies;

3. Summary of the client's progress towards achieving their habilitative goals and objectives in measurable terms

PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met by the employee to successfully perform the essential functions of this job. The employee must be able to: lift, carry, push, and/or pull items with a strength factor of light to medium work. Hear and speak to exchange information in person and on the telephone. See to read a variety of materials. Observe, listen, and speak to clients. Possess dexterity of hands and fingers to operate equipment. Bend at the waist, kneel/squat. Sit or stand for extended periods of time.
