The JFS Special NeedsSocial Worker will provide provide case management support as well as support group facilitation and navigation/linkage to other programs and resources within and outside JFSLA. Services will be provided in-person and via telehealth. Some evenings and Sundays required.

Qualifications:

• Master’s Degree in social work or related field
• Strong background in Special Needs and Special Needs resources required
• Unrestricted CA Driver’s License and proof of auto insurance

• Ability to provide targeted case management
• Ability to develop, implement and run targeted support and psycho-educational groups
• Ability to plan and oversee social and life skills activies onsite at JFSLA and within the community
• Knowledge of Jewish community and Jewish religion and customs a plus

• Knowledge of safety issues when working with at-risk clients
• Must be computer literate in MS Office applications (Word, Excel and Outlook) and the ability to learn Electronic Health Records systems and new computer applications
• Culturally sensitive and able to work with a multicultural client population
• Ability to work collaboratively as part of an integrated treatment team
• Ability to maintain professional boundaries
• Ability to utilize a strength based, non-judgmental, person-centered approach
• Self starter and ability to work independently
• Ability to effectively utilize Telehealth platforms for individual and group meetings as needed
• Stellar time management skills required
• Must pass a criminal background check (Live Scan fingerprinting) prior to beginning employment

Responsibilities/Essential Functions:

• Implement JFS Special Needs program goals and objectives
• Provide targeted case management and information & referral services to individuals with Special Needs and their families

• Identify and develop family and other informal support systems with the client
• Support JFS life skills programming together working in collaboration with the program coordinator

• Provide consultation as needed to other community partners re: Special Needs
• Develop, implement and facilitate targeted support and psycho-educational groups as well as community presentations

• Provide timely documentation, tracking and recordkeeping
• Prepare monthly statistical reports
• May be required to be a Field Work Supervisor and/or preceptor for masters level students/interns

• Other related duties as assigned

• Supervised hours towards licensure available but not guaranteed
